How to Use

Dosage

The dose varies by condition. For high blood pressure, start with 10 mg once daily, maintenance is usually 20 mg once daily. For heart failure, start with 2.5 mg once daily, maintenance is 5–35 mg once daily. After a heart attack, take 5 mg first, then another 5 mg after 24 hours, and then 10 mg once daily. For diabetic kidney problems, take 10 mg or 20 mg once daily. Your doctor will tell you the exact dose.

Method

Swallow the tablet whole with water. Take your tablet at the same time each day, with or without food. Continue taking the medicine for as long as your doctor has prescribed.

Important

The maximum usual daily dose is 35 mg, for example in heart failure. Your doctor will tell you the highest safe dose.

Possible Side Effects

COMMON
  • Headache
  • Dizziness or faintness, especially when standing up quickly
  • Diarrhea
  • Dry cough that doesn't go away
  • Vomiting
  • Kidney problems (seen in blood tests)
RARE
  • Severe allergic reaction (angioedema), appearing as swelling of the face, lips, tongue, or throat, which can cause difficulty swallowing or breathing, or swelling of the hands, feet, and ankles. Seek medical help immediately.
  • Unusual mood changes
  • Changes in finger or toe color (pale to red) or numbness/tingling
  • Changes in taste
  • Drowsiness
  • Spinning sensation (vertigo)
  • Sleep problems
  • Fast heartbeat
  • Runny nose
  • Abdominal pain or digestive problems
  • Rash or itchy skin
  • Erectile dysfunction (impotence)
  • Fatigue or weakness
  • Confusion
  • Hives (urticaria)
  • Dry mouth
  • Hair loss
  • Psoriasis (skin condition)
  • Changes in sense of smell
  • Breast enlargement in men
  • Low blood sodium levels (fatigue, headache, nausea, vomiting)
  • Sudden kidney failure
  • Sinusitis (pain and pressure in cheeks and around eyes)
  • Wheezing
  • Low blood sugar (feeling hungry, weak, sweating, fast heartbeat)
  • Lung inflammation (cough, shortness of breath, fever)
  • Yellowing of skin or whites of eyes (jaundice)
  • Liver inflammation (loss of appetite, yellowing, dark urine)
  • Pancreatitis (abdominal pain)
  • Severe skin reactions (redness, blistering, peeling)
  • Increased sweating
  • Reduced or no urine production
  • Liver failure
  • Gastroenteritis
  • Heart attack
  • Stroke

Important Warnings

  • Do not use if you are allergic to lisinopril or other similar medicines (ACE inhibitors), especially if you have had swelling of your face, lips, tongue, or throat (angioedema).
  • Do not use if you are also taking a combination medicine containing sacubitril and valsartan for heart failure, as it increases the risk of angioedema.
  • Do not use if you are more than three months pregnant, or avoid use early in pregnancy.
  • Do not use if you have diabetes or kidney problems and are taking blood pressure medicine containing aliskiren.
  • Tell your doctor if you get a persistent dry cough after starting this medicine.

  • Be careful after the first dose or a dose increase, as your blood pressure may drop too low, causing dizziness or faintness.
  • Tell your doctor if you have any of these: narrowing of heart or kidney blood vessels, thickened heart muscle, low blood pressure, or liver disease.
  • Tell your doctor if you are taking medicines that can increase the risk of a severe allergic reaction (angioedema), such as mTOR inhibitors, racecadotril, or vildagliptin.
  • This medicine is not recommended for children under 6 years old or children with severe kidney problems.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
