How to Use

Dosage

The dose depends on your age and weight. Typically, 15-20 mg per kilogram for adults and 10-15 mg per kilogram for children, given several times a day.

Method

This medicine is given as an intravenous infusion, meaning it flows into your vein through a tube over at least 60 minutes. A doctor or nurse will administer it directly into your bloodstream, not into a muscle.

Important

The maximum daily dose should not be more than 2 grams.

Possible Side Effects

COMMON
  • Low blood pressure
  • Trouble breathing, noisy breathing (like wheezing)
  • Rash, inflammation in the mouth, itching, or hives
  • Kidney problems (often found through blood tests)
  • Redness of the upper body and face, or inflamed veins
RARE
  • Temporary or permanent hearing loss
  • Decrease in white blood cells, red blood cells, or platelets (cells that help blood clot)
  • Increase in certain white blood cells
  • Balance problems, ringing in the ears, or dizziness
  • Inflammation of blood vessels
  • Feeling sick (nausea)
  • Kidney inflammation or kidney failure
  • Pain in chest and back muscles
  • Fever, chills
  • Very severe allergic skin reaction with peeling skin, blisters, or skin detachment, possibly with high fever and joint pain
  • Cardiac arrest (heart stops beating)
  • Inflammation of the intestine causing stomach pain and possibly bloody diarrhea
  • Vomiting, diarrhea
  • Confusion, drowsiness, weakness, swelling, fluid buildup, or less urine
  • Rash with swelling or pain in lymph nodes (behind ears, neck, groin, under jaw, armpits), or abnormal blood/liver tests
  • Rash with blisters and fever

Important Warnings

  • Do not use if you are allergic to vancomycin or any of its ingredients, or if you've had an allergic reaction to similar antibiotics like teicoplanin.
  • Tell your doctor if you have hearing problems (especially if you are older) or kidney problems, as your doctor may need to test your hearing and kidney function during treatment.
  • Immediately stop taking this medicine and seek medical help if you develop severe skin reactions like widespread rash, peeling skin, blisters, or sores in your mouth, throat, nose, genitals, or eyes, especially if accompanied by fever.
  • Seek immediate medical help if you experience signs of a severe allergic reaction, such as sudden wheezing, difficulty breathing, redness of the upper body, rash, or itching.
  • This medicine should be used with caution in premature and young infants because their kidneys are not fully developed, and the medicine can build up in their blood.

  • Tell your doctor about all other medicines you are taking, especially those that can harm your kidneys or hearing, as taking them with vancomycin can increase the risk of side effects.
  • Vancomycin can harm an unborn baby's ears and kidneys, and it passes into breast milk. Use during pregnancy or breastfeeding only if clearly necessary and discussed with your doctor.
