How to Use

Dosage

The usual dose is 125 mg every 6 hours; your doctor might increase it up to 500 mg every 6 hours if needed.

Method

Take the capsules by mouth, swallowing them whole with water.

Important

Do not take more than 2 grams (2000 mg) in a single day.

Possible Side Effects

COMMON
  • Low blood pressure
  • Shortness of breath, noisy breathing (a high-pitched sound due to blocked airflow in the upper airways)
  • Rash and inflammation of the mouth lining, itching, itchy rash, hives
  • Kidney problems, mainly found with blood tests
  • Redness of the upper body and face, inflammation of a vein
RARE
  • Temporary or permanent hearing loss
  • Decrease in white blood cells, red blood cells, and platelets (cells needed for blood clotting). Increase in some white blood cells.
  • Balance problems, ringing in the ears, dizziness
  • Inflammation of blood vessels
  • Nausea (feeling sick)
  • Kidney inflammation and kidney failure
  • Pain in chest and back muscles
  • Fever, chills
  • Sudden, severe allergic skin reaction causing peeling skin, blisters, or skin shedding. This might come with high fever and joint pain.
  • Cardiac arrest (heart stops beating)
  • Inflammation of the intestine causing stomach pain and possibly bloody diarrhea
  • Vomiting, diarrhea
  • Confusion, drowsiness, weakness, swelling, fluid buildup, less urine
  • Rash with swelling or pain behind the ears, in the neck, groin, under the chin, and armpits (swollen lymph nodes), abnormal blood and liver function test results
  • Rash with blisters and fever

Important Warnings

  • Do not take this medicine if you are allergic to vancomycin or any other ingredients in this medicine.
  • Tell your doctor if you have inflamed bowels or kidney problems, as you might have a higher risk of side effects.
  • Tell your doctor if you have hearing problems.
  • If you are pregnant or planning to get pregnant, ask your doctor or pharmacist before taking this medicine, as its safety during pregnancy is not fully known.
  • If you are breastfeeding, ask your doctor for advice before taking this medicine, as it passes into breast milk.

  • This medicine can sometimes cause dizziness. If you feel dizzy, do not drive or use machines.
  • Tell your doctor if you have ever had a severe rash, peeling skin, blisters, or sores in your mouth after taking vancomycin.
  • Stop taking this medicine and see a doctor right away if you get red or round patches on your body, often with blisters, skin peeling, or sores in your mouth, throat, nose, genitals, and eyes, which can be signs of serious skin reactions like Stevens-Johnson syndrome; or if you get a widespread rash, fever, and swollen lymph nodes (DRESS syndrome); or a red, scaly, widespread rash with bumps under the skin and blisters with fever at the start of treatment (AGEP).
  • Vancomycin can cause allergic reactions. Severe allergic reactions are rare but serious. Tell your doctor right away if you suddenly get wheezing, trouble breathing, redness on your upper body, a rash, or itching.
  • Contact your doctor or nurse if you experience ringing in your ears (tinnitus), as this could be an early sign of hearing loss.
