How to Use

Dosage

For adults and adolescents (12 years and older), the typical dose is 15-20 mg per kilogram of body weight, given every 8 to 12 hours. For children (1 month to under 12 years), it's 10-15 mg per kilogram every 6 hours.

Method

This medicine is given as a slow drip (infusion) directly into a vein, taking at least 60 minutes.

Important

The maximum daily dose for adults should not be more than 2 grams.

Possible Side Effects

COMMON
  • Drop in blood pressure
  • Shortness of breath, noisy breathing
  • Rash and inflammation in the mouth, itching, hives
  • Kidney problems (found with blood tests)
  • Redness in the upper body and face, inflammation of a vein
RARE
  • Temporary or permanent hearing loss
  • Decrease in white blood cells, red blood cells, and platelets; increase in certain white blood cells
  • Balance problems, ringing in the ears, dizziness
  • Inflammation of blood vessels
  • Nausea, vomiting, diarrhea, bowel inflammation (may be bloody)
  • Kidney inflammation and kidney failure
  • Chest and back muscle pain
  • Fever, chills
  • Sudden, severe allergic skin reaction (peeling, blisters, shedding, fever, joint pain)
  • Cardiac arrest
  • Confusion, drowsiness, weakness, swelling, fluid retention, reduced urine output
  • Rash with swelling or pain behind ears, neck, groin, under chin and armpits (swollen lymph nodes), abnormal blood and liver function tests
  • Rash with blisters and fever

Important Warnings

  • Do not use if you are allergic to vancomycin or teicoplanin.
  • Tell your doctor if you have hearing problems or kidney issues, or if you will be on this medicine for a long time (you may need tests).
  • If you get severe skin reactions like peeling skin, blisters, or mouth sores (such as Stevens-Johnson syndrome or DRESS syndrome), stop using the medicine right away and contact a doctor.
  • If you have severe or long-lasting diarrhea during or after treatment, contact a doctor immediately, as it could be a sign of a serious bowel infection.
  • Be careful if you are taking other medicines that can affect your kidneys or nerves (like certain antibiotics, pain relievers, or antifungals), as they can increase the risk of kidney damage.

  • Using vancomycin with anesthetics can cause skin redness and allergic reactions in children.
