How to Use

Dosage

For adults and adolescents, the typical intravenous dose is 15-20 mg per kg of body weight, given every 8-12 hours. Doses for children are based on their weight and age.

Method

This medicine is usually given by a healthcare professional as an infusion into a vein (intravenously) over at least 60 minutes. For certain stomach infections, it can be given as a liquid you drink (orally).

Important

The maximum daily dose for adults and children over 12 years is 2 grams (2000 mg).

Possible Side Effects

COMMON
  • Low blood pressure
  • Shortness of breath, noisy breathing
  • Skin rash, itching, hives, or inflammation in the mouth
  • Redness of the upper body and face, inflammation of a vein
  • Kidney problems (mainly detected by blood tests)
RARE
  • Temporary or permanent hearing loss
  • Changes in blood cell counts (e.g., fewer white or red blood cells, or platelets)
  • Dizziness, ringing in the ears, or loss of balance
  • Nausea, vomiting, diarrhea, or abdominal pain
  • Fever, chills, or pain in the chest or back muscles
  • Swelling, confusion, drowsiness, weakness, or decreased urine production
  • Cardiac arrest
  • Severe allergic skin reactions with peeling skin, blisters, high fever, and joint pain

Important Warnings

  • Do not use if you are allergic to vancomycin or similar medicines.
  • Stop taking this medicine and see a doctor right away if you get severe skin rashes, blisters, or skin peeling, especially if you also have fever or flu-like symptoms.
  • Tell your doctor if you have kidney or hearing problems, as these may need special monitoring during treatment.
  • Contact your doctor immediately if you get severe or long-lasting diarrhea during or after treatment, as this could be a sign of a serious gut infection.
  • This medicine should only be given into a vein (intravenously), not into a muscle or the eyes, to avoid serious tissue damage or vision loss.
