Vancomycin is an antibiotic that kills certain bacteria causing severe infections in different parts of the body.
The dose depends on age, weight, and infection type. For intravenous use, adults typically receive 15-20 mg/kg every 8-12 hours, and children 10-15 mg/kg every 6 hours. Oral doses for specific gut infections are 125-500 mg every 6 hours.
This medicine is usually given in a hospital as an infusion directly into a vein over at least 60 minutes. For certain gut infections, it can be given orally as a solution.
The maximum daily dose should not exceed 2 grams (2000 mg) regardless of the administration route.
