How to Use

Dosage

Adults and adolescents (12+ years): The usual dose is 15-20 mg per kilogram of body weight, given every 8 to 12 hours. The doctor may give a higher starting dose of up to 30 mg per kg in some cases.

Method

Vancomycin Bioglan is given as a slow drip (infusion) directly into a vein, not into a muscle. Each dose should be given over at least 60 minutes.

Important

The maximum daily dose should not be more than 2 grams.

Possible Side Effects

COMMON
  • Drop in blood pressure
  • Shortness of breath, noisy breathing
  • Rash and inflammation of the mouth lining, itching, itchy rash, hives
  • Kidney problems, mainly found through blood tests
  • Redness of the upper body and face, vein inflammation
RARE
  • Temporary or permanent hearing loss
  • Decrease in the number of white blood cells, red blood cells, and platelets (cells that help blood clot)
  • Increase in the number of some white blood cells
  • Balance problems, ringing in the ears, dizziness
  • Blood vessel inflammation
  • Nausea
  • Kidney inflammation and kidney failure
  • Pain in the chest and back muscles
  • Fever, chills

Important Warnings

  • Do not use if you are allergic to vancomycin or any other ingredients in this medicine.
  • Tell your doctor if you have had an allergic reaction to teicoplanin, as you might also be allergic to vancomycin.
  • Tell your doctor if you have hearing problems, especially if you are elderly, as you might need hearing tests during treatment.
  • Tell your doctor if you have kidney problems, as your blood and kidney function will need to be tested during treatment.
  • Seek immediate medical attention if you get a severe skin rash or peeling, blisters, or mouth sores after taking vancomycin.

  • Severe skin reactions, including Stevens-Johnson syndrome, toxic epidermal necrolysis, DRESS syndrome, and AGEP, have been reported. Stop using vancomycin and seek medical help right away if you notice any such symptoms.
  • Serious side effects, including vision loss, have been reported after vancomycin injection into the eyes.
  • Vancomycin should be used with extra caution in premature babies and infants because their kidneys are not fully developed, and vancomycin can build up in the blood. Blood tests may be needed to monitor levels.
  • If you are elderly, your doctor may adjust your dose because your kidneys naturally filter less effectively with age.
  • If you are pregnant, breastfeeding, or planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
  • Certain medicines, including some antibiotics, anti-inflammatory drugs (NSAIDs), and antifungal medications, can increase the risk of kidney damage when taken with vancomycin. Regular blood and kidney tests may be necessary.
