How to Use

Dosage

Use one vaginal tablet every day for the first two weeks. After that, use one vaginal tablet two times a week.

Method

Insert the tablet into your vagina using the applicator provided. Do not take it by mouth.

Important

Do not use more than one tablet per day during the first two weeks, and no more than two tablets per week thereafter.

Possible Side Effects

  • Fungal infection in the genital area
  • Feeling sick (nausea)
  • Skin rash
  • Weight gain
  • Hot flashes
  • High blood pressure
  • Diarrhea
  • Fluid retention
  • Worsening migraine
  • Severe allergic reaction (like anaphylaxis/shock)
  • Trouble sleeping
  • Itching in the vagina and genital area
  • Hives (itchy welts on the skin)
  • Allergic skin reactions at the application site, such as rash in the genital area
  • Injury related to using the applicator
  • Medication does not work
  • Breast cancer
  • Womb cancer
  • Deep vein blood clot (in leg veins)
  • Excessive thickening of the womb lining
  • Pain in the external genitals and vagina
  • Reaction at the application site
  • Gallbladder disease
  • Skin discoloration, especially on the face or neck (dark patches)
  • Painful, reddish lumps on the skin (erythema nodosum)
  • Ring-shaped, reddish, or sore rash (erythema multiforme)

Important Warnings

  • Do not use if you are allergic to estradiol or any other ingredient in Vagifem.
  • Do not use if you have or have ever had breast cancer, or if it is suspected.
  • Do not use if you have or have ever had cancer sensitive to estrogens (e.g., cancer of the womb lining), or if it is suspected.
  • Do not use if you have unexplained vaginal bleeding.
  • Do not use if you have an untreated thickening of the womb lining (endometrial hyperplasia).

  • Do not use if you have or have ever had a blood clot in a vein (e.g., in the legs or lungs).
  • Stop using Vagifem and talk to your doctor right away if you become pregnant.
  • Stop using Vagifem and talk to your doctor right away if you get a migraine-like headache for the first time.
  • Stop using Vagifem and talk to your doctor right away if your skin or the whites of your eyes turn yellow (jaundice), which could be a sign of liver problems.
  • Stop using Vagifem and talk to your doctor right away if your face, tongue, and/or throat swell, or you have difficulty swallowing or hives with breathing difficulties (signs of angioedema).
  • Stop using Vagifem and talk to your doctor right away if your blood pressure rises significantly (symptoms may include headache, tiredness, dizziness).
  • Stop using Vagifem and talk to your doctor right away if you notice signs of a blood clot, such as painful swelling and redness in your legs, sudden chest pain, or breathing difficulties.

Use in Elderly Patients

Category D

The classification depends on the dosage form. Systemic use (for example estrogen patch, tablet, gel): Avoid use in older adults. In systemic use, consider potential adverse effects, such as risk of cardiovascular diseases and venous thromboembolism. Consider potential interactions.
