How to Use

Dosage

For adults and young people over 12, the usual dose is one to two 50 mg, 100 mg, 150 mg, or 200 mg tablets twice a day, in the morning and evening, or as your doctor tells you.

Method

Swallow the tablets whole with enough water. Do not cut or chew them. You can take them with or without food, preferably in the morning and evening.

Important

Do not take more than 400 mg of tramadol hydrochloride per day, unless your doctor has specifically told you to.

  • Unusual skin feelings (e.g., tingling, numbness), shaking, muscle twitching, problems with movement, temporary loss of consciousness (fainting), speech problems
  • Seizures or fits
  • Changes in appetite
  • Seeing or hearing things that aren't there (hallucinations), confusion, trouble sleeping, anxiety, nightmares
  • Mood changes (feeling very happy or irritable), changes in how active you are, changes in thinking or how you feel things
  • Becoming dependent on the medicine
  • Blurred vision, pupils getting smaller or bigger
  • Slow or difficult breathing
  • Muscle weakness
  • Trouble or pain when urinating, less urine than usual
  • Low blood sugar
  • Hiccups

Important Warnings

  • Do not use if you are allergic to tramadol or any other part of this medicine.
  • Do not use if you have been poisoned by alcohol, sleeping pills, other pain medicines, or drugs that affect your mood.
  • Do not use if you have taken MAO inhibitors (medicines for depression) in the last 14 days.
  • Do not use if you have epilepsy that is not controlled by medicine, or if you are in treatment for opioid withdrawal.
  • Talk to your doctor before taking Tramal retard if you feel faint, are in shock (cold sweat), have high pressure in your brain, have breathing problems, or have had seizures before.

  • Tell your doctor if you have depression and are taking antidepressants, or if you have liver or kidney disease.
  • This medicine is an opioid. Using it repeatedly can lead to your body getting used to it (tolerance), dependence, abuse, and a risk of life-threatening overdose. This risk increases with higher doses and longer use.
  • Not for children under 12 or those with breathing problems, as side effects can be worse for them.
  • There's a small risk of a serious condition called serotonin syndrome, especially if you take this medicine with certain antidepressants. Get medical help right away if you have symptoms like agitation, hallucinations, fever, fast heart rate, or muscle stiffness.
  • Do not drink alcohol during treatment, as it can make the medicine work too strongly.
  • Do not use during pregnancy. If you are breastfeeding, do not use more than once, or stop breastfeeding if you use it more often.
  • This medicine can make you sleepy, dizzy, or blur your vision, making it unsafe to drive or use machines. Do not drive or operate machines if you feel your reactions are slow.

Use in Elderly Patients

Category D

Avoid use in older adults. Serotonergic. Prodrug. Genotype affects the response. Reduce the dose in moderately severe and severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. Consider potential interactions. (updated 18.8.2025)
