How to Use

Dosage

One 100 mg tablet twice daily, usually in the morning and evening. Your doctor may increase the dose if needed.

Method

Swallow the tablets whole with enough liquid; do not break or chew them. You can take them with or without food.

Important

Do not take more than 400 mg of tramadol hydrochloride per day, unless your doctor has told you to.

RARE
  • Heart and circulation problems (fast heartbeat, feeling faint, collapsing), especially when standing up quickly or during physical activity.
  • Retching (feeling like vomiting), stomach upset (e.g., pressure, bloating), diarrhea.
  • Skin reactions (e.g., itching, rash).
  • Allergic reactions (e.g., breathing difficulties, wheezing, swollen skin) and shock (sudden circulatory collapse).
  • Slow heartbeat.
  • Increased blood pressure.
  • Unusual sensations (e.g., itching, tingling, numbness), tremors, seizures, muscle spasms, involuntary movements, temporary loss of consciousness (fainting), speech difficulties.
  • Changes in appetite.
  • Hallucinations, confusion, sleep disorders, delirium, anxiety, and nightmares.
  • Mental side effects after treatment, such as mood changes (e.g., elevated or irritable mood), changes in activity (e.g., decreased or increased), and problems with thinking and senses.
  • Drug dependence and withdrawal symptoms if treatment is stopped suddenly.
  • Blurred vision, dilated pupils (mydriasis), constricted pupils (miosis).
  • Slow breathing, shortness of breath (dyspnea).
  • Muscle weakness.
  • Difficulty or pain when urinating, reduced urine volume (dysuria).
  • Increased liver enzymes.
  • Decreased blood sugar level.
  • Hiccups.
  • Serotonin syndrome, with symptoms like mental changes (agitation, hallucinations, coma), fever, fast heartbeat, unstable blood pressure, involuntary muscle twitching, muscle stiffness, coordination problems, and/or stomach issues (nausea, vomiting, diarrhea).

Important Warnings

  • Do not take if you are allergic to tramadol or any other ingredient in this medicine.
  • Do not take if you have acute poisoning from alcohol, sleeping pills, pain relievers, or medicines that affect mood.
  • Do not take if you are using MAO inhibitors (medicines for depression) or have used them within the last 14 days.
  • Do not take if you have epilepsy that is not well controlled by treatment.
  • Do not take as a substitute for opioid withdrawal treatment.

  • Talk to your doctor if you suspect you are dependent on other pain medicines (opioids).
  • Talk to your doctor if you have impaired consciousness (you feel like fainting) or are in shock (symptoms may include cold sweats).
  • Taking more than 400 mg per day can increase the risk of seizures.
  • Do not drink alcohol during treatment with Tramadol Krka, as it can increase the medicine's effects.
  • Tramadol Krka should not be used during pregnancy.
  • Do not use Tramadol Krka more than once while breastfeeding, or stop breastfeeding if you use it more often.
  • This medicine can cause drowsiness, dizziness, and blurred vision, affecting your ability to react. Do not drive or operate machinery if your reaction time is impaired.
  • Contact your doctor if you experience extreme tiredness, loss of appetite, severe stomach pain, nausea, vomiting, or low blood pressure, as these may be signs of adrenal insufficiency.
  • Stop taking this medicine and seek immediate medical attention if you notice slow or shallow breathing, confusion, drowsiness, small pupils, nausea or vomiting, constipation, or loss of appetite.
  • Contact your doctor immediately if you experience symptoms of an allergic reaction such as swelling of the face, tongue, and/or throat, difficulty swallowing, or hives with breathing difficulties.

Use in Elderly Patients

Category D

Avoid use in older adults. Serotonergic. Prodrug. Genotype affects the response. Reduce the dose in moderately severe and severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. Consider potential interactions. (updated 18.8.2025)
