Topiramat ratiopharm is a medicine used to treat epilepsy seizures in adults and children (6+ years alone, 2+ years with other medicines), and to prevent migraine headaches in adults.

How to Use

Dosage

Your doctor will start you on a low dose and slowly increase it until the right dose is found for you.

Method

Swallow tablets whole with plenty of fluids throughout the day to help prevent kidney stones. Do not chew the tablets as they taste bitter. You can take them with or without food. Avoid alcohol.

Important

Always follow your doctor's instructions for the maximum daily dose; do not exceed it.

RARE
  • Increased blood acidity (can cause breathing difficulties, loss of appetite, nausea, vomiting, severe tiredness, fast or irregular heartbeat)
  • Reduced or absent sweating (especially in small children in high temperatures)
  • Severe suicidal thoughts and attempts of self-harm
  • Partial loss of visual field
  • Glaucoma (high eye pressure, pain, or vision loss due to fluid blockage in the eye)
  • Difficulty thinking, remembering, or solving problems; reduced alertness or awareness; severe drowsiness and lack of energy (due to high ammonia levels)
  • Severe skin reactions (like Stevens-Johnson syndrome or toxic epidermal necrolysis, with rashes or blisters)
  • Decreased platelets, white blood cells, and potassium in the blood
  • Increased liver enzymes, increased eosinophils in blood
  • Swelling of glands in the neck, armpit, or groin
  • Increased appetite
  • Elevated mood
  • Hearing, seeing, or feeling things that are not there; severe mental disorder (psychosis)
  • Lack of emotions or inability to show them, unusual suspicion, panic attacks
  • Difficulty reading, speech problems, difficulty writing
  • Restlessness, hyperactivity
  • Slowed thinking, reduced alertness or awareness
  • Reduced or slow body movements, unusual or repetitive involuntary movements
  • Fainting
  • Abnormal sensation, reduced sense of touch
  • Reduced, distorted, or absent sense of smell
  • Unusual sensations that may precede migraine or certain types of epileptic seizures
  • Dry eyes, light sensitivity, involuntary eyelid twitching, watery eyes
  • Reduced or lost hearing, hearing loss in one ear
  • Slow or irregular heartbeat, pounding heart
  • Low blood pressure, low blood pressure upon standing
  • Flushing, feeling warm
  • Pancreatitis (inflammation of the pancreas)
  • Excessive gas, heartburn, feeling full or bloated in the stomach
  • Bleeding gums, increased saliva, drooling, bad breath
  • Excessive drinking, thirst
  • Skin discoloration
  • Stiff muscles, side pain
  • Blood in urine, incontinence, urgent need to urinate, side or kidney pain
  • Difficulty getting or keeping an erection, sexual dysfunction
  • Flu-like symptoms
  • Cold fingers and toes
  • Feeling drunk
  • Learning difficulties
  • Abnormally elevated mood
  • Loss of consciousness
  • Blindness in one eye, temporary blindness, night blindness
  • Poor vision
  • Swelling inside and around the eye
  • Numbness, tingling, and color changes (white, blue, then red) in fingers and toes when exposed to cold
  • Liver inflammation, liver failure
  • Unusual skin odor
  • Discomfort in arms or legs
  • Kidney dysfunction
  • Maculopathy (a disease of the macula, the part of the retina with the sharpest vision)
  • Eye inflammation (uveitis), symptoms include eye redness, pain, light sensitivity, watery eyes, seeing small spots or blurred vision

Important Warnings

  • Do not take this medicine if you are allergic to topiramate or any other ingredient in it.
  • This medicine can harm an unborn baby. Do not use if pregnant, unless absolutely necessary for epilepsy and advised by your doctor. Women who can get pregnant must use very effective birth control during treatment and for at least 4 weeks after the last dose.
  • Talk to your doctor if you have kidney problems (especially kidney stones or dialysis), liver problems, or eye problems (especially glaucoma).
  • Inform your doctor if you have blood acidity problems (metabolic acidosis) or are on a high-fat ketogenic diet.
  • This medicine can cause suicidal thoughts. If you have such thoughts, contact your doctor immediately.

  • Severe skin reactions can occur. Tell your doctor right away if you get a rash or blisters.
  • This medicine can rarely increase ammonia in your blood, which may affect brain function. If you have trouble thinking, remembering, solving problems, feel less alert, very sleepy, or have no energy, tell your doctor immediately.
  • If you have epilepsy, do not stop taking this medicine without talking to your doctor first, as your condition could worsen.
  • This medicine can cause dizziness, tiredness, and vision problems. Do not drive or use machines until you know how it affects you and have discussed it with your doctor.
