How to Use

Dosage

Your doctor will usually start treatment with a low dose and slowly increase it until the most suitable dose is found for you.

Method

Swallow the tablets whole. Do not chew them as they taste bitter. You can take Topiramat Orion before, during, or after a meal. Drink plenty of fluids daily to help prevent kidney stones while taking this medicine.

Important

The maximum daily dose will be determined by your doctor, who will slowly adjust it to find the most effective and tolerable dose for you.

Possible Side Effects

COMMON
  • Depression (new or worsening)
  • Seizures
  • Anxiety, irritability, mood changes, confusion, disorientation
  • Problems with concentration, slow thinking, forgetfulness, memory problems (new, worsening, or severe)
  • Kidney stones, frequent or painful urination
  • Stuffy or runny nose, sore throat
  • Tingling, pain, and/or numbness in parts of the body
  • Drowsiness, fatigue
  • Dizziness
  • Nausea, diarrhea
  • Weight loss
  • Anemia (low blood count)
  • Allergic reaction (e.g., rash, redness, itching, facial swelling, hives)
  • Loss of appetite, decreased appetite
  • Aggression, agitation, anger, abnormal behavior
  • Difficulty falling asleep or interrupted sleep
  • Speech problems or disorders, slurred speech
  • Clumsiness or lack of coordination, feeling unsteady when walking
  • Reduced ability to perform routine tasks
  • Decreased, lost, or absent taste
  • Involuntary tremors or shaking, rapid, uncontrolled eye movements
  • Vision problems, double vision, blurred vision, decreased vision, difficulty focusing eyes
  • Feeling of spinning (vertigo), ringing in ears, ear pain
  • Shortness of breath
  • Cough
  • Nosebleed
  • Fever, malaise, weakness
  • Vomiting, constipation, abdominal pain or discomfort, indigestion, stomach or intestinal infection
  • Dry mouth
  • Hair loss
  • Itching
  • Joint pain or swelling, muscle spasms or twitching, muscle pain or weakness, chest pain
  • Weight gain
RARE
  • Increased blood acidity (can cause breathing difficulties, loss of appetite, nausea, vomiting, severe fatigue, fast or irregular heartbeat)
  • Decreased or absent sweating (especially in small children exposed to high temperatures)
  • Severe suicidal thoughts, attempts at self-harm
  • Partial loss of visual field
  • Glaucoma (increased eye pressure, pain, vision loss)
  • Difficulty thinking, memory problems, reduced alertness, severe drowsiness, lack of energy (may indicate high blood ammonia affecting brain function)
  • Severe skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis) which can be life-threatening, causing rashes, blisters, and skin peeling.
  • Decreased platelets, decreased white blood cells, decreased blood potassium
  • Increased liver enzymes, increased eosinophils in blood
  • Swollen glands in neck, armpits, or groin
  • Increased appetite
  • Elevated mood
  • Hearing, seeing, or feeling things that aren't real; severe mental disorder (psychosis)
  • Lack of emotion, inability to show feelings, unusual suspicion, panic attack
  • Difficulty reading, speech problems, difficulty writing
  • Restlessness, hyperactivity
  • Slowed thinking, reduced alertness or awareness
  • Reduced or slowed body movements, abnormal or repetitive involuntary movements
  • Fainting
  • Abnormal sensation, reduced sense of touch
  • Decreased, distorted, or absent sense of smell
  • Unusual sensations that may precede migraine or certain types of epileptic seizures
  • Dry eyes, light sensitivity, involuntary eyelid twitching, watery eyes
  • Decreased or lost hearing, hearing loss in one ear
  • Slow or irregular heartbeat, feeling of heart pounding in chest
  • Low blood pressure, including when standing up (may cause weakness, dizziness, or fainting when suddenly standing or sitting up)
  • Flushing, feeling warm
  • Pancreatitis (inflammation of the pancreas)
  • Excessive gas, heartburn, feeling of fullness or bloating in the stomach
  • Gum bleeding, increased saliva production, drooling, bad breath
  • Excessive drinking, thirst
  • Skin discoloration
  • Muscle stiffness, side pain
  • Blood in urine, urinary incontinence, urgent need to urinate, flank or kidney pain
  • Difficulty getting or maintaining an erection, sexual dysfunction
  • Flu-like symptoms
  • Cold fingers and toes
  • Feeling intoxicated
  • Learning difficulties
  • Abnormally elevated mood
  • Loss of consciousness
  • Blindness in one eye, temporary blindness, night blindness
  • Weak vision
  • Swelling inside and around the eye
  • Numbness, tingling, and color changes (white, blue, then red) in fingers and toes when exposed to cold
  • Liver inflammation, liver failure
  • Unusual skin odor
  • Uncomfortable sensations in arms or legs
  • Kidney dysfunction

Important Warnings

  • Do not take Topiramat Orion if you are allergic to topiramate or any other ingredient in this medicine.
  • Do not use Topiramat Orion if you are pregnant or could become pregnant, unless no other effective treatment for your seizures exists and your doctor has discussed the risks with you. Women who can become pregnant must use highly effective contraception during treatment and for at least 4 weeks after the last dose.
  • Tell your doctor if you have kidney problems (especially kidney stones or dialysis), liver problems, eye problems (like glaucoma), a condition where your blood or body fluids are too acidic (metabolic acidosis), or if you are on a ketogenic diet.
  • If you have epilepsy, do not stop taking this medicine suddenly without talking to your doctor first, as this could worsen your condition.
  • Your weight may decrease while taking this medicine. Your doctor will monitor your weight regularly.

  • If you ever have thoughts of self-harm or suicide, contact your doctor immediately.
  • Topiramat Orion can cause severe skin reactions. Tell your doctor right away if you develop a rash or blisters.
  • In rare cases, Topiramat Orion may increase ammonia levels in your blood, especially if taken with valproic acid. This can affect brain function. Tell your doctor immediately if you experience difficulty thinking, memory problems, reduced alertness, or severe drowsiness.
