How to Use

Dosage

The usual dose is one tablet once a day.

Method

Take the tablet whole with water, at the same time each day, with or without food. Keep taking it daily until your doctor tells you to stop.

Important

If you have liver problems, the daily dose of telmisartan should not be more than 40 mg.

Possible Side Effects

COMMON
  • Dizziness
  • Feeling sick (nausea)
  • Increased blood fat levels
  • Low magnesium levels in blood
  • Decreased appetite
RARE
  • Low blood pressure, or a sudden drop when standing up
  • Fast, slow, or irregular heartbeat
  • Shortness of breath or difficulty breathing, bronchitis, cough, sinus inflammation, or scarring in lungs
  • Stomach problems: diarrhea, constipation, gas, stomach pain, upset stomach, inflammation of stomach lining, nausea, vomiting, dry mouth, or inflammation of the pancreas
  • Kidney problems, including acute kidney failure, urinary tract infections, glucose in urine, or high uric acid levels (which can cause gout)
  • Electrolyte changes: low or high potassium, low sodium (with possible confusion or tiredness), low magnesium, high calcium, low or high blood sugar
  • Nerve or brain problems: headache, anxiety, depression, fainting, vertigo (spinning sensation), tingling, numbness, drowsiness, trouble sleeping, or confusion
  • Vision problems: blurred vision, impaired vision, or sudden eye pain (can be serious)
  • Skin issues: redness, itching, rashes, hives, eczema, severe allergic reactions (e.g., swelling of face or throat), increased sweating, sensitivity to sun, blisters, peeling skin, or drug/toxic rash
  • Muscle or joint problems: back pain, muscle cramps, muscle pain, joint pain, or tendon pain
  • Blood problems: anemia (low red blood cells), low platelet count (increases risk of bleeding or bruising), or changes in white blood cells
  • Liver problems: abnormal liver function, yellow skin or eyes (jaundice), or excess bile in blood
  • Other: chest pain, flu-like symptoms, fever, general weakness, erectile dysfunction, worsening of lupus, or inflammation of blood vessels

Important Warnings

  • Do not take this medicine if you are allergic to telmisartan, hydrochlorothiazide, or any related medicines (sulfonamide derivatives).
  • Do not take this medicine if you are pregnant, especially after the first 3 months, as it can harm your baby. Talk to your doctor if you are pregnant or planning to be.
  • Do not take this medicine if you have very severe liver problems (like cholestasis or blocked bile ducts) or severe kidney problems (passing less than 100 ml of urine per day).
  • Do not take this medicine if your doctor found that you have low potassium or high calcium levels in your blood that are not getting better with treatment.
  • Do not take this medicine if you have diabetes or kidney problems and are also taking another blood pressure medicine called aliskiren.

  • Talk to your doctor right away if you experience sudden vision changes or eye pain. These could be signs of a serious eye problem.
  • This medicine may increase your risk of skin cancer. Protect your skin from sun and UV light while taking it, and tell your doctor about any new skin changes.
  • This medicine can make you feel dizzy or lightheaded. Do not drive or use machines if you feel this way.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Effectiveness is reduced in renal impairment. Consider risk of orthostatic hypotension and hyponatraemia and -kalemia. (updated 19.5.2025)
