How to Use

Dosage

Usually one Telmisartan Orion tablet (40 mg or 80 mg) once daily. Your doctor will tell you the exact dose.

Method

Take the tablet whole with water or any non-alcoholic drink, with or without food. Try to take it at the same time each day.

Important

If you have liver problems, the maximum dose is 40 mg per day. Otherwise, your doctor will determine the maximum daily dose.

Possible Side Effects

COMMON
  • Low blood pressure, especially when standing up quickly.
  • Urinary tract infection.
  • Upper respiratory tract infection (e.g., sore throat, sinus infection, common cold).
  • Lack of red blood cells (anemia).
  • High potassium levels in the blood.
  • Difficulty sleeping.
  • Feeling down or sad (depression).
  • Fainting (syncope).
  • Feeling dizzy or spinning (vertigo).
  • Slow heart rate (bradycardia).
  • Dizziness when standing up (orthostatic hypotension).
  • Shortness of breath.
  • Cough.
  • Stomach pain.
  • Diarrhea.
  • Upset stomach.
  • Abdominal swelling.
  • Vomiting.
  • Itching.
  • Increased sweating.
  • Rash caused by the medicine.
  • Back pain.
  • Muscle cramps.
  • Muscle pain.
  • Kidney problems, including sudden kidney failure.
  • Chest pain.
  • Feeling weak.
  • Increased creatinine levels in the blood.
RARE
  • Sepsis (a severe infection, also known as blood poisoning, that can be life-threatening).
  • Sudden severe swelling of the skin and mucous membranes (angioedema), including swelling of the face and throat, which can be life-threatening.
  • Severe allergic reactions, including rash, itching, difficulty breathing, wheezing, facial swelling, or very low blood pressure.
  • Low blood sugar levels (in diabetic patients).
  • Abnormal liver function (this side effect is more likely in Japanese patients).
  • Increase in certain white blood cells (eosinophilia), low platelet count (thrombocytopenia).
  • Feeling anxious.
  • Feeling drowsy.
  • Blurred or impaired vision.
  • Fast heart rate (tachycardia).
  • Dry mouth.
  • Altered taste (dysgeusia).
  • Eczema (a skin condition).
  • Skin redness.
  • Hives (urticaria).
  • Joint pain (arthralgia).
  • Pain in the arms or legs.
  • Tendon pain.
  • Flu-like symptoms.
  • Decreased hemoglobin levels (a protein in the blood).
  • Increased uric acid levels.
  • Increased levels of liver enzymes or creatine phosphokinase in the blood.
  • Low sodium levels in the blood.
  • Progressive scarring of lung tissue (interstitial lung disease).
  • Swelling in the intestines (intestinal angioedema), causing stomach pain, nausea, vomiting, and diarrhea.

Important Warnings

  • Do not take this medicine if you are allergic to telmisartan or any other ingredients.
  • Do not use this medicine if you are pregnant or planning to become pregnant, especially during the last 6 months of pregnancy, as it can harm your baby.
  • Do not take this medicine if you have severe liver problems or a blocked bile duct.
  • Do not take this medicine if you have diabetes or kidney problems and are also taking a blood pressure-lowering medicine containing aliskiren.
  • Tell your doctor if you have or have had kidney disease, liver disease, heart problems, very low blood pressure, high potassium levels, or diabetes.

  • Tell your doctor if you are taking other blood pressure medicines (like ACE inhibitors or aliskiren) or digoxin.
  • Tell your doctor if you are scheduled for surgery or anesthesia.
  • This medicine is not recommended for children or adolescents under 18 years.
  • If you feel dizzy or lightheaded, do not drive or operate machinery.

Use in Elderly Patients

Category A

Suitable for older adults. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
