How to Use

Dosage

0.1–2 ml for joints, depending on their size. 0.1–0.75 ml for other painful inflamed areas like bursae and tendon sheaths.

Method

The injection must be given cleanly (aseptically). Shake well before use. Do not give this medicine into a vein.

Important

The dose can be repeated every 1–5 weeks.

Possible Side Effects

COMMON
  • Infection
  • Your body may produce less of its own hormones (cortisol) if used for a long time.
  • Rounded face and weight gain.
  • Swelling due to fluid and salt build-up in the body.
  • Feeling sad or overly happy for no reason.
  • Cataract (clouding of the eye lens).
  • High blood pressure.
  • Stomach or duodenal ulcer (which may burst and bleed).
  • Swelling in arms and legs.
  • Bruises, thin and fragile skin, acne.
  • Bone loss (osteoporosis).
  • Muscle weakness.
  • Wounds heal slower.
  • Low potassium levels in the blood.
RARE
  • Severe body chemistry problems (tumor lysis syndrome) when starting cancer treatment, or a type of blood vessel cancer (Kaposi's sarcoma).
  • Infections because your immune system is weaker.
  • Increased white blood cells in your blood.
  • Allergic reactions to the medicine, including very serious ones.
  • Problems with your pituitary gland and withdrawal symptoms when stopping steroids (e.g., poor appetite, nausea, vomiting, sleepiness, headache, fever, joint pain, muscle pain, weight loss, and/or low blood pressure).
  • Problems with how your body handles sugar, too much alkalinity in blood from low potassium, fat disorders in blood, too much acid in body, increased need for insulin or diabetes medicines, nitrogen loss from protein breakdown, high urea in blood, increased calcium in urine, increased appetite (can cause weight gain), local fat deposits in the body.
  • Mood changes, mental addiction, thoughts of harming yourself, mental problems (like mania, false beliefs, seeing or hearing things that aren't there, or worsening schizophrenia), confusion, anxiety, personality changes, strange behavior, tiredness, trouble sleeping, irritability.
  • Increased pressure in the head, seizures, memory loss, problems with thinking or remembering, dizziness, headache.
  • Bulging eyes, increased eye pressure (glaucoma), eye diseases, thinning of the clear or white parts of the eye.
  • Heart failure (if you are prone to it), irregular heartbeats, heart muscle tear after a heart attack.
  • Very low blood pressure, blood clot in an artery, increased blood clotting, blood clot in the lung.
  • Stomach and digestive system issues (e.g., stomach ulcer, pancreatitis, peritonitis, esophageal inflammation, stomach pain, abdominal bloating, diarrhea, indigestion, nausea), hiccups.
  • Liver inflammation, changes in liver function tests.
  • Skin problems, such as local swelling (angioedema), small bruises, visible capillaries, skin scars, skin pigmentation changes, increased hair growth, rash, skin redness, itching, hives, skin reactions at the injection site, weaker skin test reactions, increased sweating.
  • Bone breakdown due to poor blood flow, unusual bone breaks, muscle wasting, muscle disease, joint disease from nerve problems, joint pain, muscle pain, tendon tears, spinal fractures.
  • Irregular menstrual periods.
  • Feeling generally unwell.
  • Blurred vision.
  • Severe kidney problem (scleroderma renal crisis) in patients who already have scleroderma, with symptoms like high blood pressure and less urine output.
  • This medicine can slow down growth in children.

Important Warnings

  • Do not use if you have a fungal infection that affects your whole body.
  • Do not use if you have a blood disorder called idiopathic thrombocytopenic purpura (low platelet count, causing easy bruising or bleeding) and the medicine is given into a muscle.
  • Do not use if you are allergic to methylprednisolone, other corticosteroids, bupivacaine, other amide-type anesthetics, or any other ingredients in this medicine.
  • This medicine must not be given into a vein.
  • For tendon problems, the injection should be given into the tendon sheath, not the tendon itself. This medicine is not recommended for Achilles tendon inflammation.

  • Avoid drinking grapefruit juice during treatment. Grapefruit juice can make the side effects of this medicine stronger.
  • If you are pregnant, breastfeeding, think you might be pregnant, or are planning a baby, talk to your doctor before using this medicine. It will only be given if the doctor thinks the benefits outweigh the risks for the baby.
  • This medicine may cause side effects like dizziness, vision problems, and tiredness. If you experience these, do not drive or use machines.
  • This medicine contains benzyl alcohol, which can cause allergic reactions. It is linked to serious side effects in young children, including breathing problems. Do not give to newborns (up to 4 weeks old) unless advised by a doctor. Do not give to children under 3 years old for more than a week, unless advised by a doctor. If you have liver or kidney problems, or are pregnant or breastfeeding, ask your doctor before using, as large amounts of benzyl alcohol can build up and cause problems.
