How to Use

Dosage

Typically, take 20 mg once a day, but your doctor may adjust your dose.

Method

Take one tablet once a day, with or without food. Swallow it whole with water. Do not chew the tablet.

Important

For adults, the maximum daily dose is 40 mg. For elderly patients or those with liver problems, the maximum is 20 mg per day.

Possible Side Effects

COMMON
  • Drowsiness
  • Headache
  • Nausea or vomiting
  • Dry mouth
  • Increased sweating
  • Diarrhea or constipation
  • Problems with sexual function (e.g., ejaculation, erection, orgasm)
RARE
  • Seizures (grand mal attacks)
  • Liver inflammation
  • Uncontrolled movements
  • Bleeding (e.g., unusual bruising)

Important Warnings

  • Do not take if you are allergic to citalopram or any other ingredients in this medicine.
  • Do not take if you are currently taking or have recently taken MAOI medicines (e.g., moclobemide, selegiline, linezolid). There are specific waiting periods before starting or stopping Sepram with MAOIs.
  • Do not take if you have a known abnormal heart rhythm (seen on an ECG) or if you are taking medicines for heart rhythm problems.
  • If you experience suicidal thoughts or thoughts of harming yourself, especially at the start of treatment or if you are young, contact your doctor immediately or go to the nearest hospital.
  • This medicine is generally not recommended for children and adolescents under 18 years old due to increased risk of side effects like suicidal thoughts and aggression.

  • Seek immediate medical attention if you experience high fever, restlessness, confusion, tremors, or sudden muscle jerks, as these could be signs of a rare but serious condition called serotonin syndrome.
  • Get urgent medical help if you have swelling of the skin, tongue, lips, or face, or difficulty breathing or swallowing (signs of a severe allergic reaction).
  • Report any unusual bleeding, including from the stomach or intestines, to your doctor.
  • Contact your doctor immediately if you experience nausea, vomiting, muscle twitching, and confusion, as these might indicate low sodium levels (hyponatremia).
  • Seek urgent medical advice if you experience a fast, irregular heartbeat or fainting, as these could be symptoms of a life-threatening heart condition (Torsades de Pointes).

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Serotonergic. Risk of QT prolongation. A significant adverse effect is low serum sodium level. Increases risk of falls. Consider potential interactions especially with preparations that increase risk of bleeding. (updated 12.2.2024)
