How to Use

Dosage

Your doctor will tell you the exact dose. For preventing blood clots, adults usually take one 20 mg capsule once a day. For treating blood clots, adults typically take one 15 mg capsule twice a day for 3 weeks, then one 20 mg capsule once a day. Doses for children are based on weight.

Method

Take the capsule(s) with food and swallow them whole with water. If you can't swallow them, you can mix the capsule contents with water or applesauce and eat it right away. Try to take your medicine at approximately the same time each day.

Important

Do not take more than 30 mg (two 15 mg capsules) in one day for adults. For children, follow your doctor's instructions strictly.

Possible Side Effects

COMMON
  • Reduced red blood cells, which can cause pale skin, weakness, or shortness of breath.
  • Bleeding in the stomach, intestines, urinary or genital areas (including blood in urine and heavy menstrual bleeding), nosebleeds, gum bleeding.
  • Eye bleeding (including white parts of the eye).
  • Bleeding into tissue or body cavities (bruising).
  • Coughing up blood.
  • Bleeding from or under the skin.
  • Bleeding after surgery.
  • Oozing blood or fluid from a surgical wound.
  • Swelling in arms or legs.
  • Pain in limbs.
  • Kidney problems (seen in doctor's tests).
  • Fever.
  • Stomach pain, indigestion, nausea or vomiting, constipation, diarrhea.
  • Low blood pressure (may cause dizziness or fainting when standing up).
  • Reduced general strength and energy (weakness, tiredness), headache, dizziness.
  • Rash, itchy skin.
  • Blood tests may show increased liver enzymes.
RARE
  • Bleeding in the brain or inside the head.
  • Bleeding into a joint causing pain and swelling.
  • Low platelet count (cells that help blood clot).
  • Allergic reactions, including skin reactions.
  • Liver problems (seen in doctor's tests).
  • Fainting.
  • Fast heart rate.
  • Bleeding into a muscle.
  • Gallbladder problems (reduced bile flow), liver inflammation (including liver damage).
  • Yellowing of the skin and eyes (jaundice).
  • Local swelling.
  • Eosinophilic lung inflammation (a type of white blood cell causing lung inflammation).
  • Kidney failure after severe bleeding.
  • Increased pressure in leg or arm muscles after bleeding, causing pain, swelling, numbness or paralysis (compartment syndrome).

Important Warnings

  • Do not take if you are allergic to rivaroxaban or any other ingredient in this medicine.
  • Do not take if you are currently bleeding a lot.
  • Do not take if you have an illness or condition that increases your risk of severe bleeding, like a stomach ulcer or recent brain/eye surgery.
  • Do not take if you have a severe liver disease that increases your bleeding risk.
  • Do not take if you are pregnant or breastfeeding.

  • Tell your doctor immediately if you notice signs of bleeding, such as a severe headache, weakness on one side of your body, vomiting, seizures, or a stiff neck.
  • Tell your doctor immediately if you notice a severe skin rash, blisters, or sores in your mouth or eyes (signs of severe skin reactions).
  • Tell your doctor immediately if you have swelling of the face, lips, mouth, tongue or throat, difficulty swallowing, hives, shortness of breath, or a sudden drop in blood pressure (signs of a severe allergic reaction).

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Consider renal impairment and body weight when determining the dosage. Consider potential interactions especially with other drugs that increase the risk of bleeding. (updated 19.9.2025)
