How to Use

Dosage

The usual starting dose is one 1 mg tablet, taken three times a day for two weeks. Your doctor will adjust the dose to find what works best for you.

Method

Take the tablets three times a day, every 6-8 hours, with or without food. If you have trouble swallowing, you can crush the tablet and mix it with water or soft food like applesauce just before taking it.

Important

Do not take more than 2.5 mg three times a day, which is a total maximum daily dose of 7.5 mg.

  • Headache

  • Swelling in arms and legs
  • Diarrhea
  • Nausea or vomiting
  • Coughing up blood
  • Inflammation of the stomach lining (gastritis)
  • Inflammation of the stomach and intestines (gastroenteritis)
  • Low red blood cells (anemia), which may cause pale skin, weakness, or shortness of breath
  • Irregular, strong, or fast heartbeat (palpitations)
  • Low blood pressure (hypotension)
  • Nosebleeds
  • Difficulty breathing through the nose (blocked nose)
  • Stomach, bowel, or abdominal pain
  • Heartburn (reflux disease)
  • Difficulty swallowing
  • Constipation
  • Abdominal swelling (bloating)
RARE
  • Sudden bleeding in the lungs, which can be very serious or even fatal

Important Warnings

  • Do not take this medicine if you are allergic to riociguat or any other ingredient in it.
  • Do not take if you are using medicines called PDE5 inhibitors (e.g., sildenafil, tadalafil, vardenafil), which are used for high blood pressure in the lungs or erection problems.
  • Do not take if you have severe liver disease.
  • Do not take if you are pregnant. You should also take a monthly pregnancy test.
  • Do not take if you are using nitrates or nitric oxide donor medicines (like amyl nitrite), including recreational "poppers." These are used for high blood pressure, chest pain, or heart disease.

  • Do not take if you are using other soluble guanylate cyclase (sGC) stimulators, like vericiguat.
  • Do not take if you have low blood pressure before starting this treatment (systolic blood pressure below 90 mmHg for children 6-12 years, or below 95 mmHg for patients 12 years and older).
  • Do not take if you have high blood pressure in your lungs from an unknown cause with lung scarring (idiopathic pulmonary pneumonia).
  • Tell your doctor if you recently had severe bleeding from your lungs or have been treated for coughing up blood.
  • Tell your doctor if you are taking blood thinners (anticoagulants), as this medicine can increase the risk of bleeding in your lungs. Your doctor will monitor you regularly.
  • Tell your doctor if you feel short of breath, which might be due to fluid buildup in your lungs.
  • Tell your doctor if you have symptoms of low blood pressure (e.g., dizziness, fainting), or if you take blood pressure medicines, water pills, or have heart or circulation problems.
  • Tell your doctor if you are taking medicines for fungal infections (e.g., ketoconazole) or HIV infection (e.g., abacavir), as your doctor may need to adjust your dose.
  • Do not use this medicine if you are on dialysis or have severe kidney problems (creatinine clearance less than 30 ml/min).
  • Tell your doctor if you have moderate liver problems.
  • Tell your doctor if you start or stop smoking during treatment, as this can affect how the medicine works and your dose may need to be adjusted.
