How to Use

Dosage

The recommended starting dose is one 1 mg tablet 3 times a day for 2 weeks.

Method

Take tablets three times a day, every 6–8 hours, with or without food. They can be crushed and mixed with water or soft food.

Important

Your doctor may gradually increase the dose up to a maximum of 2.5 mg 3 times a day, making the maximum daily dose 7.5 mg.

Possible Side Effects

COMMON
  • Coughing up blood (hemoptysis)
  • Headache
  • Dizziness
  • Indigestion (dyspepsia)
  • Swelling of arms and legs (peripheral edema)
  • Diarrhea
  • Nausea or vomiting
  • Inflammation of the stomach (gastritis)
  • Inflammation of the digestive tract (gastroenteritis)
  • Decrease in red blood cells (anemia), showing as pale skin, weakness, or shortness of breath
  • Irregular, strong, or fast heartbeat (palpitations)
  • Low blood pressure (hypotension)
  • Nosebleed (epistaxis)
  • Difficulty breathing through the nose (nasal congestion)
  • Stomach, intestinal, or abdominal pain
  • Heartburn (reflux disease)
  • Difficulty swallowing (dysphagia)
  • Constipation
  • Abdominal swelling
RARE
  • Sudden bleeding in the lungs (pulmonary hemorrhage)

Important Warnings

  • Do not take this medicine if you are allergic to riociguat or any other of its ingredients.
  • Do not take if you are already using medicines called PDE5 inhibitors (like sildenafil).
  • Do not take if you have severe liver disease.
  • Do not take if you are pregnant.
  • Do not take if you are using medicines called nitrates or nitric oxide donors.

  • Do not take if you are using other similar medicines that stimulate soluble guanylate cyclase (like vericiguat).
  • Do not take if you have very low blood pressure.
  • Do not take if you have high blood pressure in your lungs due to unknown reasons, with lung scarring.
  • Tell your doctor if you have recently had serious bleeding in your lungs.
  • Tell your doctor if you have been treated for coughing up blood.
  • Tell your doctor if you are taking blood thinning medicines, as this medicine can increase bleeding risk.
  • Tell your doctor if you feel very short of breath.
  • Tell your doctor if you have symptoms of low blood pressure like dizziness or fainting.
  • Tell your doctor if you are taking medicines for fungal infections (e.g., ketoconazole) or HIV infection (e.g., ritonavir), as your dose may need to be adjusted.
  • Tell your doctor if you are on dialysis or your kidneys do not work well, as this medicine is not recommended.
  • Tell your doctor if you have moderate liver problems.
  • Tell your doctor if you start or stop smoking during treatment, as your dose may need to be changed.
  • Women who can get pregnant must use effective birth control during this treatment.
