How to Use

Dosage

Start by taking one 1 mg tablet three times a day for two weeks. Your doctor may slowly increase your dose every two weeks, up to a maximum of 2.5 mg three times a day.

Method

Take tablets three times a day, about every 6-8 hours, with or without food. If you have a tendency for low blood pressure, do not switch between taking it with and without food. You can crush the tablet and mix it with water or soft food like applesauce right before taking it.

Important

The maximum daily dose is 7.5 mg (2.5 mg three times a day). Your doctor will determine the best dose for you.

Possible Side Effects

RARE
  • Sudden lung bleeding, which can lead to coughing up blood, sometimes severely.
COMMON
  • Coughing up blood (hemoptysis).
  • Headache.
  • Dizziness.
  • Indigestion (upset stomach after meals).
  • Swelling in arms and legs (peripheral edema).
  • Diarrhea.
  • Nausea or vomiting.
  • Stomach inflammation (gastritis).
  • Inflammation of the digestive tract (gastroenteritis).
  • Low red blood cells (anemia), which may cause pale skin, weakness, or shortness of breath.
  • Fast or irregular heartbeat (palpitations).
  • Low blood pressure (hypotension).
  • Nosebleed.
  • Difficulty breathing through the nose (nasal congestion).
  • Stomach or abdominal pain.
  • Heartburn (reflux disease).
  • Difficulty swallowing.
  • Constipation.
  • Abdominal bloating.

Important Warnings

  • Do not take if you are allergic to riociguat or any other ingredient in this medicine.
  • Do not take if you are using medicines called PDE5 inhibitors (e.g., sildenafil, tadalafil, vardenafil), which are used for high blood pressure in the lungs or erection problems.
  • Do not take if you have severe liver disease.
  • Do not take if you are pregnant. You should also take monthly pregnancy tests.
  • Do not take if you are using nitrates or nitric oxide donor medicines (e.g., amyl nitrite), including recreational drugs known as 'poppers'.

  • Do not take if your blood pressure is low before starting this treatment.
  • Tell your doctor if you have recently had severe bleeding from your lungs or have been treated for coughing up blood.
  • Tell your doctor if you are taking blood thinners, as this medicine can increase the risk of bleeding in the lungs.
  • Tell your doctor if you feel short of breath, which might be due to fluid buildup in your lungs.
  • Tell your doctor if you start or stop smoking during treatment, as this can affect how well the medicine works.
