How to Use

Dosage

Start with one 1 mg tablet, three times a day for two weeks. Your doctor may slowly increase your dose later.

Method

Take tablets three times a day, about 6–8 hours apart. You can take them with or without food. If you have low blood pressure, do not switch between taking with and without food. If you have trouble swallowing, you can crush the tablet and mix it with water or soft food like apple sauce right before taking.

Important

The maximum daily dose is 7.5 mg (2.5 mg taken three times a day).

Possible Side Effects

COMMON
  • Headache
  • Dizziness
  • Upset stomach after eating (indigestion)
  • Swelling in arms and legs
  • Diarrhoea
  • Feeling sick (nausea) or throwing up (vomiting)
  • Stomach inflammation (gastritis)
  • Gut inflammation (gastroenteritis)
  • Low red blood cells (anemia), which can make you look pale, feel weak, or short of breath
  • Irregular, strong, or fast heartbeat (palpitations)
  • Low blood pressure
  • Nosebleed
  • Difficulty breathing through the nose (nasal congestion)
  • Pain in your stomach, gut, or belly
  • Heartburn (reflux disease)
  • Difficulty swallowing
  • Constipation
  • Bloated belly (gas)
  • Coughing up blood (hemoptysis)
RARE
  • Sudden severe bleeding in the lungs, which can be fatal.

Important Warnings

  • Do not take this medicine if you are allergic to riociguat or any other ingredient.
  • Do not take this medicine if you are pregnant or think you might be pregnant; you should also get a pregnancy test monthly.
  • Do not take this medicine if you have very low blood pressure or severe liver disease.
  • Do not take this medicine if you are taking other specific medicines for high blood pressure in the lungs or erectile dysfunction (like sildenafil or tadalafil), or medicines called nitrates or nitric oxide donors (including 'poppers').
  • Do not take this medicine if you have high blood pressure in your lungs due to unknown reasons and lung scarring.

  • Tell your doctor if you have had serious bleeding from your lungs recently or have been treated for coughing up blood.
  • Tell your doctor if you take blood thinners, as this medicine can increase the risk of lung bleeding.
  • Tell your doctor if you feel very short of breath due to fluid in your lungs.
  • Tell your doctor if you have symptoms of low blood pressure like dizziness or fainting, or if you take medicines for blood pressure or heart problems.
  • Inform your doctor if you start or stop smoking during treatment, as your dose may need to be adjusted.
  • Discuss with your doctor if you are on dialysis or have kidney or moderate liver problems.
