How to Use

Dosage

Your doctor will determine your starting dose, usually 1.25 mg or 2.5 mg once daily.

Method

Take the tablets by mouth with liquid at the same time each day. Do not crush or chew. Tablets can be divided.

Important

The maximum daily dose is 10 mg. For heart failure or post-heart attack, it may be recommended to take the dose twice daily.

Possible Side Effects

COMMON
  • Headache or feeling tired.
  • Dizziness, especially when starting or increasing the dose.
  • Fainting, unusually low blood pressure, especially when standing or sitting up quickly.
  • Dry, irritating cough, sinus infection (sinusitis) or bronchitis, shortness of breath.
  • Stomach or gut pain, diarrhea, indigestion, feeling or being sick.
  • Skin rash, with or without swelling.
  • Chest pain.
  • Muscle cramps or muscle pain.
  • Higher than normal potassium levels in the blood.
RARE
  • Balance problems (vertigo).
  • Itching and unusual skin sensations like numbness, tingling, prickling, burning or crawling (paresthesia).
  • Changed or lost sense of taste.
  • Sleep problems.
  • Feeling low, anxious, nervous or restless.
  • Blocked nose, difficulty breathing or worsening of asthma.
  • Swelling in the gut (intestinal angioedema) causing abdominal pain, vomiting, and diarrhea.
  • Heartburn, constipation or dry mouth.
  • More urine than usual every day.
  • Worsening kidney function, acute kidney failure.
  • More sweating than usual.
  • Loss or decrease of appetite.
  • Faster or irregular heartbeats, swollen arms and legs (signs of fluid buildup).
  • Flushing.
  • Blurred vision.
  • Joint pain.
  • Fever.
  • Inability to get an erection (men), reduced sex drive (men and women).
  • Increased number of certain white blood cells (eosinophilia) seen in blood tests.
  • Blood test changes showing problems with liver, pancreas, or kidney function.
  • Feeling shaky or confused.
  • Red and swollen tongue.
  • Severe skin peeling or flaking, itchy, lumpy rash.
  • Nail problems (e.g., loose or detached nails).
  • Skin rash or bruising.
  • Patches on the skin and cold limbs.
  • Red, itchy, swollen or watery eyes.
  • Hearing problems or ringing in the ears.
  • Feeling weak.
  • Blood tests showing fewer red blood cells, white blood cells, platelets, or lower hemoglobin.
  • Increased sensitivity to sunlight.
  • Concentrated (dark-colored) urine, nausea or vomiting, muscle cramps, confusion and seizures, possibly due to inappropriate ADH secretion. Contact a doctor immediately if these symptoms occur.
  • Difficulty concentrating.
  • Mouth ulcers.
  • Blood tests showing increased antibody levels.
  • Blood tests showing too few blood cells.
  • Blood tests showing lower than normal sodium levels.
  • Discoloration of fingers and toes when cold, followed by tingling and pain when warmed (Raynaud's phenomenon).
  • Breast enlargement (in men).
  • Slower or weaker reactions.
  • Burning sensation.
  • Changes in sense of smell.
  • Hair loss.

Important Warnings

  • If you are allergic to ramipril, another ACE inhibitor, or any other ingredient in this medicine.
  • If you have ever had a severe allergic reaction called 'angioedema', which involves itching, hives, red patches, and swelling of the hands, feet, throat, tongue, eyes, and lips, making it hard to breathe or swallow.
  • If you are undergoing dialysis or any other blood filtering treatment, as Ramipril ratiopharm may not be suitable depending on the equipment used.
  • If you have a kidney condition where blood flow to your kidneys is reduced (renal artery stenosis).
  • If you are in the last 6 months of pregnancy.

  • If your blood pressure is unusually low or unstable; your doctor will assess this.
  • If you have diabetes or kidney problems and are being treated with a blood pressure-lowering medicine containing aliskiren.
  • If you are taking or have recently taken sacubitril/valsartan, a medicine for chronic heart failure, as it increases the risk of angioedema (rapid swelling under the skin, e.g., in the throat).

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
