How to Use

Dosage

The usual starting dose is 1.25 mg or 2.5 mg once a day.

Method

Take the tablets whole with liquid, at the same time each day. Do not crush or chew them.

Important

The maximum daily dose is 10 mg.

Possible Side Effects

COMMON
  • Headache or feeling tired
  • Feeling dizzy, especially when starting the medicine or increasing the dose
  • Fainting or unusually low blood pressure, especially when standing or sitting up quickly
  • Dry, irritating cough, sinus infection (sinusitis) or bronchitis, shortness of breath
  • Stomach or gut pain, diarrhea, indigestion, feeling sick or nausea
  • Rash, possibly with swelling
  • Chest pain
  • Cramps or muscle pain
  • Blood tests showing higher than normal potassium levels
RARE
  • Balance problems (vertigo)
  • Itching and unusual skin sensations like numbness, tingling, prickling, burning or strange feelings on the skin
  • Loss or change of taste
  • Sleep problems
  • Feeling down, worried, restless, or more nervous
  • Stuffy nose, difficulty breathing, or worsening asthma
  • Swelling in the gut (intestinal angioedema) with stomach pain, vomiting, and diarrhea
  • Heartburn, constipation, or dry mouth
  • More urination than usual
  • More sweating than usual
  • Loss or decrease of appetite
  • Increased or irregular heartbeats
  • Swollen arms or legs, which can be a sign of fluid buildup
  • Flushing
  • Blurred vision
  • Joint pain
  • Fever
  • Inability to have sex (men), reduced sex drive (men and women)
  • Increased number of certain white blood cells (eosinophilia) seen in blood tests
  • Blood tests showing changes in liver, pancreas, and kidney function
  • Feeling uncertain or confused
  • Red and swollen tongue
  • Severe skin peeling or flaking, itchy, lumpy rash
  • Nail problems (like partial or complete detachment from the nail bed)
  • Rash or bruises
  • Patches on the skin and cold limbs
  • Red, itchy, swollen, or watery eyes
  • Hearing problems or ringing in the ears
  • Feeling weak
  • Blood tests showing lower red blood cell, white blood cell, platelet, or hemoglobin values
  • Increased sensitivity to sunlight
  • Difficulty concentrating
  • Swelling of the mouth
  • Blood tests showing too few blood cells
  • Blood tests showing lower than normal sodium in the blood
  • Concentrated urine (dark), nausea, vomiting, muscle cramps, confusion, and seizures, possibly due to a hormone imbalance. Seek medical help immediately if you have these symptoms.
  • Change in color of fingers and toes when cold, followed by tingling and pain when warming up (Raynaud's phenomenon)
  • Breast enlargement in men
  • Slowed or weakened reactions
  • Burning sensation
  • Changes in sense of smell
  • Hair loss

Important Warnings

  • Do not take this medicine if you are allergic to ramipril, any other ACE inhibitor, or any other ingredient in this medicine. Signs of an allergic reaction can include rash, difficulty swallowing or breathing, and swelling of your lips, face, throat, or tongue.
  • Do not take this medicine if you have ever had a severe allergic reaction called angioedema. Symptoms include itching, hives, red patches on hands, feet, and throat, swelling of the throat, tongue, around the eyes and lips, and difficulty breathing or swallowing.
  • Do not take this medicine if you are using or have recently used a combination medicine containing sacubitril/valsartan, which is used for a certain type of long-term heart failure. This increases the risk of angioedema (sudden swelling under the skin, e.g., in the throat).
  • Do not take this medicine if you are on dialysis or any other type of blood filtering treatment, as Ramipril Orion may not be suitable depending on the equipment used.
  • Do not take this medicine if you have kidney problems with reduced blood flow to your kidneys (renal artery narrowing).

  • Do not take this medicine during the last 6 months of pregnancy.
  • Do not take this medicine if your blood pressure is unusually low or unstable, as your doctor will evaluate this.
  • Do not take this medicine if you have diabetes or kidney problems and are being treated with a blood pressure-lowering medicine that contains aliskiren.
  • Do not take this medicine if you are using racecadotril (for diarrhea), medicines for organ transplant rejection or cancer (e.g., temsirolimus, sirolimus, everolimus), or vildagliptin (for diabetes), as the risk of angioedema may be higher.
  • Stop taking this medicine and seek immediate medical help if you experience swelling of your face, lips, or throat, leading to difficulty swallowing or breathing, along with itching and rash. These could be signs of a severe allergic reaction.
  • Seek immediate medical help if you develop severe skin reactions, including rash, mouth sores, worsening of existing skin disease, redness, blisters on the skin, or peeling skin (like Stevens-Johnson syndrome).
  • Tell your doctor immediately if you experience a faster heartbeat, uneven or strong heartbeats, chest pain, chest tightness, or more serious problems like a heart attack or stroke.
  • Tell your doctor immediately if you experience shortness of breath or a cough, as these could be signs of lung problems.
  • Tell your doctor immediately if you bruise more easily, bleed longer than usual, have signs of bleeding (like bleeding gums), purple spots on the skin, get infections more easily, have a sore throat or fever, feel tired, dizzy, or have pale skin. These could be signs of blood or bone marrow problems.
  • Tell your doctor immediately if you experience severe stomach pain that can spread to your back. This could be a sign of pancreatitis (inflammation of the pancreas).
  • Tell your doctor immediately if you have fever, chills, tiredness, loss of appetite, stomach pain, nausea, or yellowing of the skin or eyes (jaundice). These could be signs of liver problems, such as hepatitis or liver damage.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
