How to Use

Dosage

The usual starting dose is 1.25 mg or 2.5 mg once a day. Your doctor will adjust the dose.

Method

Take the tablet by mouth at the same time each day. Swallow the tablets whole with liquid; do not crush or chew them.

Important

The maximum daily dose is 10 mg.

Possible Side Effects

COMMON
  • Headache or feeling tired
  • Dizziness (more likely when starting or increasing dose)
  • Fainting or low blood pressure (especially when standing up quickly)
  • Dry, irritating cough, inflamed sinuses or bronchitis, shortness of breath
  • Stomach or gut pain, diarrhea, indigestion, feeling sick or being sick
  • Skin rash, with or without swelling
  • Chest pain
  • Muscle cramps or pain
  • Higher potassium levels in blood tests
RARE
  • Severe allergic reaction with swelling of face, lips, throat causing difficulty swallowing or breathing, itching and rashes. (STOP medication and see doctor immediately)
  • Severe skin reactions including rash, mouth sores, worsening skin conditions, redness, blisters or peeling skin (e.g., Stevens-Johnson syndrome). (STOP medication and see doctor immediately)
  • Increased heart rate, irregular or strong heartbeats, chest pain, tightness in the chest, or more severe problems like heart attack or stroke.
  • Shortness of breath or cough (signs of lung problems).
  • Easy bruising, longer bleeding, bleeding from gums, purple spots on skin, frequent infections, sore throat, fever, tiredness, dizziness, pale skin (signs of blood or bone marrow problems).
  • Severe stomach pain that may spread to your back (sign of pancreatitis).
  • Fever, chills, tiredness, loss of appetite, stomach pain, feeling sick, yellowing of skin or eyes (jaundice) (signs of liver problems like hepatitis).
  • Balance problems (vertigo)
  • Itching and unusual skin sensations (numbness, tingling, burning)
  • Loss or change of taste
  • Sleep problems
  • Feeling depressed, anxious, restless, or more nervous
  • Blocked nose, breathing difficulties, or worsening asthma
  • Swelling in the gut (intestinal angioedema) with symptoms like stomach pain, vomiting, and diarrhea
  • Heartburn, constipation, or dry mouth
  • More frequent urination than usual or increased sweating
  • Loss or decrease of appetite
  • Increased or irregular heartbeats
  • Swollen arms or legs (a sign of fluid retention)
  • Flushing
  • Blurred vision
  • Joint pain or fever
  • Inability to have sex (men), reduced sex drive (men and women)
  • Increased number of certain white blood cells (eosinophilia) in blood tests
  • Blood tests showing changes in how your liver, pancreas, or kidneys are working
  • Increased sensitivity to sunlight
  • Difficulty concentrating
  • Swelling in the mouth
  • Blood tests showing too few blood cells (red, white, platelets) or low hemoglobin
  • Blood tests showing lower than normal sodium in your blood
  • Concentrated urine (dark colour), feeling sick or being sick, muscle cramps, confusion and fits (signs of inappropriate ADH secretion)
  • Colour changes in fingers and toes when cold, then tingling or pain when warm (Raynaud's phenomenon)
  • Breast enlargement in men
  • Slowed or impaired reactions
  • Burning sensation
  • Change in sense of smell
  • Hair loss

Important Warnings

  • Do not take if you are allergic to ramipril, other ACE inhibitors, or any other ingredients. Signs include rash, swelling of face, lips, throat, or trouble breathing.
  • Do not take if you have ever had a severe allergic reaction called angioedema (e.g., itching, hives, swelling of hands, feet, throat, eyes).
  • Do not take if you are using or have recently used sacubitril/valsartan (for chronic heart failure), as it increases angioedema risk.
  • Do not take if you are on dialysis or other blood filtering treatments; Ramipril Orion may not be suitable.
  • Do not take if you have kidney problems with narrowed blood vessels to your kidneys.

  • Do not take during the last 6 months of pregnancy. If you become pregnant, tell your doctor immediately.
  • Do not take if your blood pressure is unusually low or unstable.
  • Do not take if you have diabetes or kidney problems and are taking aliskiren.
  • The risk of angioedema may be higher if you are taking racecadotril, temsirolimus, sirolimus, everolimus, or vildagliptin.
  • Tell your doctor if you have heart, liver, or kidney problems.
  • Tell your doctor if you have lost a lot of body salts or fluids (e.g., from vomiting, diarrhea, heavy sweating, low-salt diet, or diuretics).
  • Tell your doctor if you are having treatment for allergies (desensitization therapy for bee/wasp stings).
  • Tell your doctor if you are going to have surgery or dental work, as you might need to stop this medicine for a day.
  • Tell your doctor if you have high potassium levels in your blood.
  • This medicine can cause dizziness, especially at the start or with dose increases. Do not drive or use machines if you feel dizzy.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
