Qsiva is a medicine that combines two active ingredients, phentermine and topiramate, to help you lose weight and keep it off by reducing your appetite when used with a low-calorie diet and exercise.

How to Use

Dosage

Your doctor will start you on a low dose: 1 capsule of Qsiva 3.75 mg/23 mg once a day for 14 days, after which the usual dose is 1 capsule of Qsiva 7.5 mg/46 mg once a day.

Method

Swallow the capsules whole with a glass of water or another sugar-free drink in the morning, with or without food; do not crush or chew them.

Important

The maximum daily dose is 1 capsule of Qsiva 15 mg/92 mg once a day; if you need to stop this highest dose, your doctor will advise you to gradually reduce it over at least one week.

  • Swelling or dehydration
  • Increased hunger
  • Strong mood changes like confusion, anger, or feeling very restless
  • Sleep problems, abnormal dreams, or nightmares
  • Panic attacks, paranoia
  • Memory loss, drowsiness, or feeling very sluggish
  • Uncontrolled shaking, speech problems, fainting
  • Eye pain, light sensitivity, seeing flashes, double vision, itchy eyes
  • Ringing in the ears, ear pain, or deafness
  • Changes in heart rate or blood pressure, cough, nosebleeds, breathing difficulties, stomach problems like acid reflux or vomiting
  • Skin problems like rashes, itching, acne, dry skin, or sweating more than usual
  • Muscle and joint pain, muscle cramps or weakness
  • Kidney stones, trouble urinating, or urinating more often at night
  • Sexual problems (e.g., erectile dysfunction) or changes in menstrual periods
  • Suicidal thoughts, aggression, or hallucinations

Important Warnings

  • Do not take Qsiva if you are allergic to phentermine, topiramate, or any other ingredients in the medicine.
  • Do not take Qsiva if you are allergic to certain medicines called 'sympathomimetic amines' used for asthma, nasal congestion, or eye problems.
  • Women who are pregnant or could become pregnant must not take Qsiva, as it can harm an unborn baby; use highly effective birth control during treatment and for 4 weeks after your last dose.
  • Do not take Qsiva if you have used MAO inhibitor medicines (for depression or Parkinson's disease) in the last 14 days.
  • Do not take Qsiva with other medicines that help you lose weight.

  • Talk to your doctor if you have mood problems, depression, or suicidal thoughts, as Qsiva can make these worse.
  • Talk to your doctor if you have heart problems, as Qsiva can increase your heart rate; your doctor will check your heart rate regularly.
  • Drink plenty of water every day to help prevent kidney stones, which Qsiva can increase the risk of.
  • If you have sudden vision problems or eye pain, stop taking Qsiva and contact your doctor right away; these could be signs of serious eye conditions.
  • Qsiva is not recommended if you have severe liver problems, end-stage kidney disease, dialysis, or an overactive thyroid.
  • Phentermine in Qsiva can make you feel more energetic or excited, which means there's a risk of misuse or becoming dependent on the medicine.
  • Avoid alcohol while taking Qsiva, as it can increase the risk of side effects.
