How to Use

Dosage

Start with 0.26 mg once daily for the first week. Your doctor will slowly increase your dose every 5-7 days until your symptoms are controlled, usually to 1.05 mg per day, but can go up to a maximum of 3.15 mg daily if needed, or stay at 0.26 mg if sufficient.

Method

Take one tablet once a day at the same time, with or without food. Swallow the tablet whole with water. Do not chew, break, or crush the tablet as this could lead to an overdose.

Important

The maximum daily dose is 3.15 mg of pramipexole.

Possible Side Effects

COMMON
  • Sleepiness
  • Dizziness
  • Dyskinesia (uncontrolled, jerky movements)
  • Nausea (feeling sick)
  • Insomnia (trouble sleeping)
  • Hallucinations (seeing, hearing, or feeling things that are not there)
  • Abnormal dreams
  • Confusion
  • Strong urge to act in unusual ways
  • Fatigue (feeling very tired)
  • Fluid retention (swelling, usually in the legs)
  • Headache
  • Hypotension (low blood pressure)
  • Constipation
  • Vomiting (being sick)
  • Impaired vision (worsening eyesight)
  • Weight decrease, including loss of appetite
RARE
  • Delusions
  • Paranoia (e.g., excessive worry about one's health)
  • Restlessness
  • Delirium (reduced awareness, confusion, loss of contact with reality)
  • Excessive daytime sleepiness and sudden sleep attacks
  • Memory loss
  • Hyperkinesia (increased movements and inability to stay still)
  • Fainting
  • Weight increase
  • Allergic reactions (e.g., rash, itching)
  • Heart failure (heart problems that can cause shortness of breath or swollen ankles)
  • Inappropriate secretion of antidiuretic hormone (a hormone imbalance)
  • Shortness of breath
  • Hiccups
  • Pneumonia (lung infection)
  • Uncontrolled urges to do things that could harm yourself or others, such as strong gambling urge, changed or increased sexual interest/behavior, uncontrolled excessive shopping, or binge/compulsive eating.
  • Mania (agitation, elevated mood, over-excitement)
  • Spontaneous erection

Important Warnings

  • Do not take if you are allergic to pramipexole or any other ingredients.
  • Tell your doctor if you have kidney disease.
  • You may experience hallucinations (seeing, hearing, or feeling things that aren't there), mostly visual.
  • You may have dyskinesia (abnormal, uncontrolled movements), especially if you have advanced Parkinson's and are also taking levodopa.
  • Dystonia (difficulty keeping your body and neck straight) may occur, such as your head or lower back tilting forward, or your back curving to the side.

  • You may feel sleepy or fall asleep suddenly; do not drive or use machines if this happens.
  • Psychosis (symptoms like schizophrenia) can occur.
  • Vision problems may occur; your eyes should be checked regularly during treatment.
  • You may experience low blood pressure when standing up, especially at the start of treatment; your blood pressure should be checked regularly.
  • You or your family/caregiver may notice strong urges or desires to behave in unusual ways that could harm you or others, such as gambling, overeating, excessive spending, or unusual sexual urges. Tell your doctor immediately.
  • You or your family/caregiver may notice symptoms of mania (agitation, elevated mood, over-excitement) or delirium (confusion, reduced awareness of reality).
  • If you stop or reduce the dose of Pramipexole Orion, you may develop symptoms like depression, apathy, anxiety, tiredness, sweating, or pain. Contact your doctor if these symptoms persist.
  • Parts of the tablets may sometimes pass into your stool and appear whole. Inform your doctor if you see tablet pieces in your stool.
  • Be careful with alcohol while taking this medicine.
  • If you are pregnant or breastfeeding, or planning to become pregnant, talk to your doctor. Do not take this medicine during pregnancy unless your doctor tells you to. Do not use during breastfeeding.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Sedative. Reduce the dose in moderately severe renal impairment. Common adverse effects include orthostatic hypotension, confusion, sudden episodes of deep sleep, impulse control disorders and lower limb edema.. . (updated 6.10.2025)
