How to Use

Dosage

The usual starting dose is 4 mg once a day. For high blood pressure, this may be increased to 8 mg once a day. For stable heart disease, the dose may be increased to 8 mg after two weeks if well tolerated.

Method

Take the tablet with a glass of water, preferably in the morning before breakfast.

Important

The maximum recommended daily dose is 8 mg.

Possible Side Effects

COMMON
  • Headache
  • Dizziness or light-headedness
  • Spinning sensation (vertigo)
  • Tingling or pricking sensation in hands and feet
  • Blurred vision
  • Ringing or other unusual noises in the ears (tinnitus)
  • Cough
  • Shortness of breath
  • Stomach problems (like nausea, vomiting, stomach pain, changes in taste, indigestion, diarrhea, constipation)
  • Allergic reactions (like skin rash, itching)
  • Muscle cramps
  • Feeling weak or tired
RARE
  • Mood changes
  • Sleep problems
  • Depression
  • Dry mouth
  • Severe itching or serious skin rash
  • Blisters on the skin
  • Kidney problems
  • Erectile dysfunction (impotence)
  • Increased sweating
  • Increase in certain white blood cells (eosinophils)
  • Drowsiness
  • Fainting
  • Pounding or fast heartbeat
  • Inflammation of blood vessels (vasculitis)
  • Increased sensitivity to sunlight
  • Joint pain
  • Muscle pain
  • Chest pain
  • Generally feeling unwell
  • Swelling in arms or legs
  • Fever
  • Falls
  • Changes in lab test results (e.g., high potassium, low sodium, low blood sugar, increased kidney function markers)
  • Worsening of psoriasis
  • Dark urine, nausea, vomiting, muscle cramps, confusion, and seizures (can be signs of SIADH)
  • Reduced or no urine output
  • Flushing (redness of skin)
  • Sudden kidney failure
  • Confusion
  • Rare lung inflammation (eosinophilic pneumonia)
  • Runny or stuffy nose
  • Changes in blood cells (low white or red blood cells, low hemoglobin, low platelets)
  • Discoloration, numbness, and pain in fingers or toes (Raynaud's phenomenon)

Important Warnings

  • Do not take this medicine if you are allergic to perindopril or any other ACE inhibitor medicine.
  • Stop taking this medicine and get medical help right away if you experience swelling of your face, lips, tongue, or throat, or have difficulty breathing.
  • Do not take if you are pregnant or planning to become pregnant, as this medicine can seriously harm your baby.
  • Do not take if you have diabetes or kidney problems and are also taking a blood pressure medicine called aliskiren.
  • Do not take if you are currently taking sacubitril/valsartan for heart failure, as it increases the risk of severe swelling.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
