How to Use

Dosage

Adults and young people typically take 200-400 mg three times a day; children over 5 years take 200 mg three times a day.

Method

Swallow the tablet whole with at least half a glass of water or other drink, while sitting or standing. It is best to take it with food to avoid stomach problems and to drink plenty of fluids to prevent inflammation of the esophagus.

Important

For adults and young people, the maximum recommended daily dose is 1200 mg (400 mg taken three times a day). For children over 5 years, it is 600 mg (200 mg taken three times a day).

  • Hives (urticaria)
  • Itching
  • Fatigue
  • Symptoms such as fatigue and muscle weakness due to decreased carnitine levels
  • Decreased number of white blood cells
  • Drug-induced fever
  • Candida superinfection (yeast infection)

Important Warnings

  • Do not use if you are allergic to pivmecillinam or any other ingredients in this medicine.
  • Do not use if you are allergic to penicillin or cephalosporin antibiotics.
  • Do not use if you have ever had a severe rash, peeling skin, blisters, or mouth sores after using Penomax or other similar antibiotics.
  • Do not use if you have a known narrowing of the esophagus or blockages in your digestive tract.
  • Do not use if you have a known hereditary metabolic disorder that reduces the amount of carnitine in your body.

  • Do not use if you have a known hereditary metabolic disorder that causes too much acid in your blood (acidemia) or urine (aciduria).
  • Talk to your doctor if you have porphyria (a rare metabolic disorder).
  • Talk to your doctor if you have carnitine deficiency.
  • Talk to your doctor if you have unstable diabetes or unusually low muscle mass.
  • Contact your doctor immediately if you experience severe diarrhea during Penomax treatment, as it might be a sign of colitis.
  • Stop Penomax and get urgent medical help if you develop severe allergic reactions (e.g., sudden rash, swelling of face/lips/tongue/throat, difficulty breathing, fainting) or severe skin reactions (e.g., widespread rash, peeling skin, blisters, mouth sores, fever).
  • If you have taken Penomax just before childbirth, inform your doctor as it may affect newborn screening tests for metabolic disorders and give a false positive result, though there is no risk to the baby.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Increase the dosing interval in moderately severe renal impairment. May potentiate the effect of methotrexate. (updated 6.11.2023)
