How to Use

Dosage

The first two injections (150 mg, then 100 mg) are given in your upper arm one week apart. After that, you will receive an injection (25-150 mg) in your upper arm or buttock once a month. Your doctor will determine your exact dose.

Method

A doctor or other healthcare professional will give you this medicine as an injection deep into a muscle in your arm or buttock. It must not be given into a vein or under the skin.

Important

This medicine is given monthly, so there isn't a maximum daily dose. Your doctor will adjust your monthly dose based on your symptoms and kidney function.

Possible Side Effects

COMMON
  • Difficulty sleeping or staying asleep
  • Flu-like symptoms, urinary tract infection, general feeling of illness
  • Higher levels of a hormone called prolactin in blood (can cause breast swelling, sexual problems in men; breast discomfort, milk leakage, missed periods in women)
  • High blood sugar, weight gain or loss, decreased appetite
  • Irritability, depression, anxiety
  • Parkinsonism (slow movements, stiff or jerky muscles, tremors, increased saliva, flat facial expression)
  • Restlessness, drowsiness or reduced alertness
  • Dystonia (slow or prolonged involuntary muscle contractions, often in face, eyes, mouth, tongue, or jaw)
  • Dizziness
  • Involuntary muscle movements (repetitive, spastic, twisting, or jerking)
  • Tremor (shaking)
  • Headache
  • Fast heart rate
  • High blood pressure
  • Cough, stuffy nose
  • Abdominal pain, vomiting, nausea, constipation, diarrhea, indigestion, toothache
  • Increased liver enzyme levels in blood
  • Bone or muscle pain, back pain, joint pain
  • Missed menstrual periods
  • Breast milk leakage
  • Fever, weakness, fatigue
  • Injection site reaction (itching, pain, swelling)
RARE
  • Pneumonia, bronchitis, respiratory infection, sinus infection, bladder infection, ear infection, nail fungus, tonsillitis, skin infection, eye infection, skin inflammation from mites, pus under the skin
  • Low white blood cells, low platelets, anemia
  • Allergic reaction
  • New or worsening diabetes, increased insulin levels, life-threatening complications of uncontrolled diabetes, low blood sugar
  • Increased appetite or malnutrition and weight loss
  • High blood triglycerides and cholesterol
  • Sleep disorders, high mood (mania), reduced sex drive, nervousness, nightmares, confusion, sleepwalking, lack of emotions, inability to orgasm
  • Tardive dyskinesia (involuntary, jerky movements of face, tongue, or other body parts)
  • Fainting, restless legs, dizziness when standing, concentration problems, speech difficulties, loss or change of taste, reduced sensation to pain/touch, tingling or numbness
  • Blurred vision, eye inflammation or redness, dry eyes, glaucoma, eye movement problems, eye rolling, light sensitivity, increased tears, red eyes
  • Vertigo (spinning sensation), ringing in the ears, ear pain
  • Heart conduction problems, prolonged QT interval, fast or slow heart rate, abnormal EKG, palpitations
  • Low blood pressure, low blood pressure when standing up (can cause weakness, dizziness, or fainting)
  • Shortness of breath, congested airways, wheezing, sore throat, nosebleeds, sleep apnea, lung congestion, crackling breath sounds
  • Abdominal discomfort, gastroenteritis, difficulty swallowing, dry mouth, excessive gas, pancreatitis, swollen tongue, fecal incontinence, very hard stools, chapped lips
  • Increased liver enzyme GGT
  • Hives, itching, rash, hair loss, eczema, dry skin, skin redness, acne, drug rash, skin thickening, dandruff
  • Muscle breakdown and pain (rhabdomyolysis)
  • Joint swelling
  • Inability to urinate (urinary retention)
  • Breast discomfort, enlarged breasts, vaginal discharge
  • Very low body temperature, chills, feeling thirsty
  • Withdrawal symptoms
  • Pus at injection site, deep skin infection, injection site cyst, injection site bruise

Important Warnings

  • Do not use if you are allergic to paliperidone, risperidone, or any other ingredients in this medicine.
  • Seek immediate medical help if you develop blood clots, especially in your legs (swelling, pain, redness), as they can travel to your lungs causing chest pain and breathing difficulties.
  • Contact your doctor right away if you experience sudden changes in mental state, weakness or numbness on one side of your face or body, or slurred speech, as these can be signs of a stroke.
  • Get urgent medical attention if you have fever, stiff muscles, sweating, or reduced awareness, as these could be signs of a serious condition called Neuroleptic Malignant Syndrome.
  • If you are a man and experience a prolonged or painful erection (priapism), seek immediate medical help.

  • Tell your doctor if you develop uncontrolled movements of your tongue, mouth, or face (tardive dyskinesia), as your treatment may need to be stopped.
  • Seek immediate help if you have a severe allergic reaction with fever, swelling of your mouth, face, lips, or tongue, trouble breathing, itching, rash, or sometimes low blood pressure.
  • Inform your eye doctor if you are planning eye surgery, as this medicine can affect your iris during the operation.
  • Tell your doctor if you know you have a dangerously low number of certain white blood cells that fight infection.
  • Avoid alcohol while using this medicine.
  • Do not use this medicine if you are pregnant or breastfeeding unless specifically advised by your doctor, as it may harm the baby.
