How to Use

Dosage

If you haven't taken strong pain medicines before, you will usually start with 4 mg every 12 hours. Your doctor will change the dose to best control your pain.

Method

Swallow the capsule whole with water. You can also open the capsule and mix its contents with soft food, like yogurt, and eat it right away. Do not crush or chew the contents. Always take this medicine by mouth. Never inject the contents into a vein, as this can be very dangerous and even deadly.

Important

Do not take a dose more often than every 12 hours.

Possible Side Effects

  • Feeling dizzy

  • Constipation (hard stools or difficulty passing them)
  • Feeling sick to your stomach (nausea)

  • Vomiting (throwing up)

  • Feeling unusually weak
  • Not wanting to eat (loss of appetite)
  • Feeling anxious
  • Feeling confused
  • Trouble sleeping
RARE
  • Muscle twitching or spasms
  • Shaking (tremors)
  • Tingling or numbness
  • Blurred vision or other eye problems
  • Low blood pressure
  • Trouble breathing or feeling short of breath
  • Diarrhea
  • Changes in taste
  • Difficulty urinating (emptying your bladder)
  • Problems with erections (erectile dysfunction)
  • Skin rash
  • Withdrawal symptoms (when stopping the medicine)
  • Feeling tired or weak
  • Generally feeling unwell
  • Swelling in your hands, ankles, or feet
  • Bad liver test results (in blood tests)
  • Feeling agitated or restless
  • Depression
  • Feeling very happy (euphoria)
  • Seeing or hearing things that aren't there (hallucinations)
  • Bad dreams (nightmares)
  • Sedation (calm and relaxed state)
  • Very slow and weak breathing (respiratory depression)
  • Fast heartbeat
  • Pauses in breathing during sleep (sleep apnea)
  • Seizures or fits
  • Increased sensitivity to pain
  • Muscles twitching by themselves
  • Small pupils in the eyes
  • Bowel blockage (paralytic ileus)
  • Hives (itchy red bumps on skin)
  • Medicine stops working as well over time (drug tolerance)
  • Redness of the skin
  • Feeling uneasy or uncomfortable
  • Withdrawal symptoms in newborn babies

Important Warnings

  • Do not use this medicine if you are allergic to hydromorphone or any other ingredient in it.
  • Do not use if you have severe breathing problems.
  • Do not use if you have sudden, sharp stomach pain.
  • Do not use if you have too much pressure in your brain (which causes bad headaches or makes you feel sick).
  • Do not use if you are taking or have taken certain depression or Parkinson's medicines (called MAO inhibitors) in the last 2 weeks.

  • Do not use if your bowels are blocked or move very slowly (a condition called paralytic ileus).
  • Children under 12 years old should not take this medicine.
  • This medicine can make you sleepy. Do not drive or use machines if you feel drowsy, especially when you start taking it or after a dose change.
  • Do not drink alcohol with this medicine. Alcohol can make you more sleepy and increase the risk of serious side effects, like very slow breathing or passing out.
  • Do not use if you are pregnant. This medicine can cause withdrawal symptoms in babies if used for a long time during pregnancy.
  • Do not use if you are breastfeeding.
  • Taking this medicine repeatedly can lead to dependence and drug abuse, which could result in a life-threatening overdose. Talk to your doctor if you are concerned about becoming dependent.
  • Do not stop taking this medicine suddenly. Your doctor will tell you how to slowly lower your dose to avoid feeling sick from withdrawal.
  • Using this medicine with certain calming medicines (like benzodiazepines) can make you very sleepy, cause serious breathing problems, or lead to a coma. Your doctor will carefully watch you and limit the dose if you need to take both.
  • This medicine can cause breathing problems while you sleep, like stopping breathing for short times (sleep apnea) or having low oxygen. If you or someone else notices these symptoms, tell your doctor.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Sedative. Reduce the initial dose in moderately severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. (updated 24.3.2025)
