How to Use

Dosage

Take as prescribed by your doctor, usually 4 mg every 12 hours for those new to opioids. Your doctor will adjust the dose as needed.

Method

Swallow the capsule whole with water. You can also open the capsule and mix its contents with soft food like yogurt, but do not crush or chew. Take orally; never inject the contents.

Important

The maximum daily dose is set by your doctor to achieve pain relief, not more often than every 12 hours.

Important Warnings

  • Do not use if you are allergic to hydromorphone hydrochloride or any other ingredient in this medicine.
  • Tell your doctor if you have breathing difficulties or lung disease.
  • Do not use if you have severe stomach pain.
  • Do not use if you have increased pressure in your brain (severe headache, nausea).
  • Do not use if you are taking or have taken MAO inhibitors (for depression) in the last 2 weeks.

  • Do not use if you have paralytic ileus (bowel blockage where intestines don't move).
  • Children under 12 years old should not use Palladon.
  • Tell your doctor if you have severe kidney or liver problems.
  • Repeated use can lead to dependence and abuse; discuss concerns with your doctor.
  • This medicine can cause sleep-related breathing problems like sleep apnea. Tell your doctor if you experience breathing pauses during sleep or feel very sleepy during the day.
  • Do not drink alcohol while taking this medicine, as it can cause severe side effects like shallow breathing and unconsciousness.
  • Do not use during pregnancy or while breastfeeding.
  • This medicine can make you drowsy or affect your reaction time; do not drive or operate machinery if you feel this way.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Sedative. Reduce the initial dose in moderately severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. (updated 24.3.2025)
