How to Use

Dosage

Adults and young people over 12 years old typically start with a 4 mg dose every 12 hours, unless your doctor advises differently. The dose will be adjusted until pain relief is achieved. Not recommended for children under 12. Elderly patients or those with kidney/liver issues may need lower doses.

Method

Swallow the capsule whole with water. You can also open the capsule and sprinkle its contents onto soft food, like yogurt, and eat it immediately. Do not crush or chew the capsule contents. Never inject the capsule contents into a vein, as this can cause serious, potentially fatal side effects.

Important

Do not take more often than every 12 hours. Always follow your doctor's specific dosing instructions.

Important Warnings

  • Do not use if you are allergic to hydromorphone hydrochloride or any other ingredient in this medicine.
  • Do not use if you have severe breathing problems or other respiratory conditions that significantly hinder breathing.
  • Do not use if you have acute stomach pain.
  • Do not use if you have increased pressure in your brain (e.g., severe headache, nausea).
  • Do not use if you are taking or have taken monoamine oxidase inhibitors (MAOIs, medicines for depression) within the last 2 weeks.

  • Do not use if you have paralytic ileus (a condition where your bowel movements are severely slowed or stopped).
  • This medicine is not recommended for children under 12 years of age.
  • Do not use during pregnancy or while breastfeeding.
  • This medicine can make you drowsy and affect your ability to drive or use machines. Do not drive or operate machinery if you feel tired or less alert.
  • Taking this medicine with sedatives or alcohol can increase the risk of drowsiness, severe breathing problems, and coma, which can be life-threatening. Avoid alcohol completely.
  • Repeated use can lead to dependence and abuse, with a risk of life-threatening overdose. Talk to your doctor if you are concerned about becoming dependent.
  • Do not stop taking this medicine suddenly without talking to your doctor, as it can cause withdrawal symptoms.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Sedative. Reduce the initial dose in moderately severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. (updated 24.3.2025)
