How to Use

Dosage

Your doctor will give you the first two injections, 150 mg and then 100 mg, about one week apart, into your upper arm. After that, you will get a monthly injection (25-150 mg) in your upper arm or buttock. Your doctor will adjust the monthly dose based on your needs.

Method

This medicine is given as an injection into a muscle (either the upper arm or buttock) by a doctor or nurse. Do not inject it yourself.

Important

The maximum dose for a single monthly injection is 150 mg.

Possible Side Effects

COMMON
  • Difficulty falling asleep or interrupted sleep
  • Flu-like symptoms, urinary tract infection, feeling unwell
  • High hormone levels (prolactin) in the blood, which can cause breast swelling, sexual issues, or menstrual changes
  • High blood sugar, weight gain, weight loss, decreased appetite
  • Irritability, depression, anxiety
  • Slow or difficult movements, stiff muscles, tremor, increased drooling, mask-like face
  • Feeling restless, sleepy, or less alert; involuntary muscle contractions, especially in the face
  • Dizziness, involuntary jerky movements, tremor, headache
  • Fast heart rate, high blood pressure
  • Cough, stuffy nose
  • Abdominal pain, vomiting, nausea, constipation, diarrhea, indigestion, toothache
  • Increased liver enzymes in blood
  • Bone or muscle pain, back pain, joint pain
  • Missed periods
  • Fever, weakness, tiredness
  • Pain, itching, or swelling at the injection site
RARE
  • Serious infections like pneumonia, bronchitis, bladder infection, ear infection, or skin infection
  • Low white blood cell count, anemia (low red blood cells), low platelets (cells that help stop bleeding)
  • Severe allergic reaction with rash, swelling of face, mouth, or throat, and breathing difficulty
  • New or worsening diabetes, very high insulin levels, life-threatening diabetes complications, diabetic coma
  • Changes in appetite or mood (increased/decreased appetite, mania, low sex drive, nervousness, nightmares, confusion, sleepwalking, inability to have orgasm)
  • Uncontrolled jerky movements of the face, tongue, or other body parts (tardive dyskinesia)
  • Fainting, Neuroleptic Malignant Syndrome (high fever, stiff muscles, confusion), stroke, seizures, balance problems
  • Blurred vision, eye infections, dry eyes, spinning sensation, ringing in ears, earache, glaucoma (eye pressure), eye movement problems, light sensitivity, red eyes
  • Abnormal heart rhythms, slow or fast heart rate, EKG changes
  • Blood clots in lungs (chest pain, breathing problems) or legs (swelling, pain, redness)
  • Pancreatitis (inflammation of the pancreas), tongue swelling, severe constipation, bowel blockage
  • Hives, severe skin rashes, hair loss, eczema, dry skin, acne, dandruff, deep skin infections, cysts or tissue death at injection site
  • Urinary incontinence, difficulty urinating, prolonged erection (priapism), breast pain or enlargement, vaginal discharge
  • Low body temperature, chills, thirst, falls, changes in walking

Important Warnings

  • Do not use this medicine if you are allergic to paliperidone, any other ingredients in this medicine, or risperidone (another similar antipsychotic medicine).
  • If you are an older adult with dementia, this medicine may increase your risk of stroke or death.
  • Get medical help right away if you get blood clots (swelling, pain, redness in a leg), stroke symptoms (sudden weakness, confusion), high fever with stiff muscles (Neuroleptic Malignant Syndrome), prolonged painful erection, or a severe allergic reaction (rash, swelling, breathing problems).
  • Your doctor will check your weight and blood sugar, as this medicine can cause weight gain or lead to new or worsening diabetes.
  • If you are having eye surgery for cataracts, tell your eye doctor that you are taking this medicine.

  • Do not use this medicine if you are pregnant or breastfeeding without discussing it with your doctor.
  • This medicine can make you feel dizzy or tired, or cause vision problems. Be careful when driving or using machines.
