How to Use

Dosage

The first two injections (150 mg and 100 mg) are given one week apart. After that, you will receive one injection (25–150 mg) once a month. Your doctor may adjust your dose based on your symptoms.

Method

The first two injections are given into your upper arm. After that, monthly injections are given into your upper arm or buttocks. This medicine is given by a doctor or other healthcare professional.

Important

Your doctor will determine your maximum dose for each injection, typically up to 150 mg monthly after the initial doses.

Possible Side Effects

COMMON
  • Difficulty falling asleep or interrupted sleep
  • Flu-like symptoms, urinary tract infection, feeling unwell
  • Increased levels of a hormone called prolactin in your blood (may cause breast swelling, erection problems or other sexual problems in men; breast discomfort, milk leakage, missed periods or other menstrual cycle problems in women)
  • High blood sugar, weight gain, weight loss, decreased appetite
  • Irritability, depression, anxiety
  • Parkinsonism (slow movements, difficulty moving, muscle stiffness, shaking at rest, increased salivation or drooling, blank facial expression)
  • Restlessness, sleepiness or decreased alertness
  • Dystonia (slow or prolonged involuntary muscle contractions, often affecting facial muscles, causing abnormal eye, mouth, tongue, or jaw movements)
  • Dizziness
  • Involuntary muscle movements (repetitive, spastic, twisting, or twitching movements)
  • Tremor
  • Headache
  • Fast heartbeat
  • High blood pressure
  • Cough, stuffy nose
  • Abdominal pain, vomiting, nausea, constipation, diarrhea, indigestion, toothache
  • Increased liver enzymes in blood
  • Bone or muscle pain, back pain, joint pain
  • Missed menstrual periods
  • Fever, weakness, tiredness (fatigue)
  • Injection site reactions, such as itching, pain, or swelling
RARE
  • Pneumonia, bronchitis, respiratory infection, sinus infection, bladder infection, ear infection, nail fungus, tonsillitis, skin infection
  • Decreased white blood cells (which fight infections), anemia
  • Allergic reaction
  • New or worsening diabetes, increased insulin in blood
  • Increased appetite
  • Loss of appetite leading to malnutrition and weight loss
  • High triglycerides (fats) in blood, high cholesterol in blood
  • Sleep disturbances, elevated mood (mania), decreased sex drive, nervousness, nightmares
  • Tardive dyskinesia (involuntary twitching or jerking movements of the face, tongue, or other body parts)
  • Fainting, strong urge to move a body part, dizziness when standing, trouble concentrating, speech difficulties, loss or change of taste, reduced sensation of pain or touch, tingling or numbness of skin
  • Blurred vision, eye infection or redness, dry eyes
  • Sensation of spinning (vertigo), ringing in ears, ear pain
  • Heart conduction problems, prolonged QT interval on EKG, fast heart rate when standing up, slow heart rate, abnormal EKG, fluttering or pounding in chest (palpitations)
  • Low blood pressure, low blood pressure when standing up (may cause weakness, dizziness, or fainting when standing up quickly)
  • Shortness of breath, sore throat, nosebleed
  • Abdominal discomfort, stomach or intestinal inflammation, difficulty swallowing, dry mouth, excessive gas
  • Increased GGT levels (liver enzyme), increased liver enzymes in blood
  • Hives, itching, rash, hair loss, eczema, dry skin, skin redness, acne, pus pocket under skin
  • Increased creatine phosphokinase (CPK) in blood (an enzyme released during muscle breakdown)
  • Muscle cramps, joint stiffness, muscle weakness
  • Urinary incontinence (inability to hold urine), frequent urination, painful urination
  • Erectile dysfunction, ejaculation problems, missed periods or other menstrual cycle problems (women), breast development in men, sexual dysfunction, breast pain, milk discharge from breasts
  • Swelling of face, mouth, eyes, or lips, swelling of body, arms, or legs
  • Increased body temperature
  • Changes in walking style
  • Chest pain, chest discomfort, malaise
  • Skin hardening
  • Falls
  • Eye infection
  • Skin inflammation caused by mites, flaky and itchy scalp or skin
  • Increased eosinophils (a type of white blood cell) in blood
  • Decreased platelet count (platelets are blood cells that stop bleeding)
  • Head tremor
  • Inappropriate secretion of hormone regulating urine volume
  • Sugar in urine
  • Life-threatening complications of poorly controlled diabetes
  • Low blood sugar levels
  • Excessive water drinking
  • Immobility and unresponsiveness while awake (catatonia)
  • Confusion
  • Sleepwalking
  • Lack of emotion
  • Inability to have an orgasm
  • Neuroleptic malignant syndrome (confusion, decreased or lost consciousness, high fever and severe muscle stiffness), cerebrovascular disorders (including stroke or temporary reduced blood flow to the brain), unresponsiveness to stimuli, loss of consciousness, decreased level of consciousness, seizures, balance problems
  • Abnormal coordination
  • Glaucoma (high pressure in the eye)
  • Eye movement disorders, eye rolling, light sensitivity, increased tear production, red eyes
  • Atrial fibrillation (a type of irregular heartbeat), heart rhythm disorders
  • Pulmonary embolism (blood clot in the lung) causing chest pain and breathing difficulties; seek immediate medical attention if you notice these symptoms
  • Blood clots, especially in leg veins (symptoms include swelling, pain, and redness in legs); contact your doctor immediately if you notice these symptoms
  • Redness of face and neck
  • Breathing difficulties during sleep (sleep apnea)
  • Lung congestion, airway congestion
  • Wheezing, whistling breathing
  • Pancreatitis, tongue swelling, fecal incontinence, constipation (very hard stools)
  • Bowel obstruction
  • Chapped lips
  • Drug rash, skin thickening, dandruff
  • Muscle fiber breakdown and muscle pain (rhabdomyolysis)
  • Joint swelling
  • Urinary retention
  • Breast discomfort, breast gland enlargement, breast enlargement
  • Vaginal discharge
  • Priapism (prolonged erection that may require surgery)
  • Very low body temperature, chills, feeling thirsty
  • Drug withdrawal syndrome
  • Abscess at injection site, deep skin infection, cyst at injection site, bruise at injection site

Important Warnings

  • Tell your doctor immediately if you get a blood clot, especially in your legs (symptoms include swelling, pain, and redness in the leg), as it can travel to the lungs, causing chest pain and breathing difficulties.
  • If you have dementia and experience a sudden change in mental state, or sudden weakness or numbness in your face, arms, or legs (especially on one side), or slurred speech, these could be signs of a stroke.
  • Seek immediate medical help if you have a fever, muscle stiffness, sweating, or a decreased level of consciousness (a condition called neuroleptic malignant syndrome).
  • If you are a man and experience a prolonged or painful erection (priapism), seek immediate medical attention.
  • If you have involuntary, rhythmic movements of your tongue, mouth, or face, your doctor may need to stop your paliperidone treatment.

  • If you experience a severe allergic reaction with symptoms like fever, swelling of the mouth, face, lips, or tongue, shortness of breath, itching, rash, or sometimes a drop in blood pressure.
  • Inform your eye doctor if you are scheduled for eye surgery, especially for cataracts, as this medicine can affect your iris during the operation and potentially cause eye damage.
  • If you know you have dangerously low levels of certain white blood cells needed to fight infections.
