How to Use

Dosage

Initial doses are 150 mg and 100 mg, given about a week apart. After that, you will receive 25-150 mg once a month.

This medicine is given as an injection by your doctor or another healthcare professional.

Important

The maximum dose for a single monthly injection is 150 mg, adjusted by your doctor based on your symptoms.

Possible Side Effects

COMMON
  • Difficulty falling asleep or interrupted sleep.
  • Flu-like symptoms, urinary tract infection.
  • Increased prolactin hormone levels (may cause breast swelling in men/women, erection problems in men, menstrual cycle problems in women, milk discharge from breasts).
  • High blood sugar, weight gain or loss, decreased appetite.
  • Irritability, depression, anxiety.
  • Parkinsonism (slow movement, stiff muscles, tremor, increased salivation, expressionless face).
  • Restlessness, drowsiness, or feeling less alert.
  • Dystonia (slow or long-lasting involuntary muscle contractions, especially in the face, eyes, mouth, tongue, or jaw).
  • Dizziness.
  • Involuntary movements (repetitive, spastic, writhing or jerky muscle movements).
  • Tremor.
  • Headache.
  • Rapid heart rate, high blood pressure.
  • Cough, nasal congestion.
  • Abdominal pain, vomiting, nausea, constipation, diarrhea, indigestion, toothache.
  • Increased liver enzyme levels in blood.
  • Bone or muscle pain, back pain, joint pain.
  • Missed periods.
  • Fever, weakness, fatigue.
  • Injection site reactions (itching, pain, swelling).
RARE
  • Other infections like pneumonia, bronchitis, respiratory infection, sinus infection, bladder infection, ear infection, nail fungus infection, tonsillitis, skin infection, eye infection, skin inflammation caused by mites, flaky and itchy scalp or skin.
  • Blood disorders like decreased white blood cell count, anemia, increased eosinophils, decreased platelet count.
  • Severe allergic reaction.
  • Metabolic/endocrine issues like onset or worsening of diabetes, increased blood insulin, increased appetite, loss of appetite leading to malnutrition/weight loss, high triglycerides/cholesterol, sugar in urine, life-threatening complications of poorly controlled diabetes, low blood sugar, excessive water drinking, inappropriate hormone secretion regulating urine volume, very low body temperature, chills, thirst.
  • Psychiatric/nervous system issues like sleep disorders (mania, decreased sexual desire, nervousness, nightmares), tardive dyskinesia (jerky, uncontrolled movements of face/tongue/body), fainting, strong urge to move body part, dizziness when standing, concentration disorder, speech difficulties, loss or abnormality of taste, weakened pain/touch sensations, tingling/numbness, head tremor, immobility/unresponsiveness (catatonia), confusion, sleepwalking, lack of emotions, inability to orgasm, neuroleptic malignant syndrome (confusion, decreased consciousness, high fever, muscle stiffness), coordination abnormalities.
  • Eye disorders like blurred vision, red eyes, dry eyes, spinning sensation (vertigo), eye movement disorders, eye rolling, light sensitivity, increased tear production, glaucoma (increased eye pressure).
  • Ear disorders like ringing in the ears, ear pain.
  • Cardiovascular issues like heart rhythm disorders (QT prolongation, rapid/slow heart rate, ECG abnormalities, palpitations), low blood pressure (especially when standing up), cerebrovascular disorders (including stroke), atrial fibrillation, pulmonary embolism, blood clots (especially in legs).
  • More severe respiratory issues like shortness of breath, sore throat, nosebleed, breathing difficulties during sleep (sleep apnea), pulmonary congestion, airway congestion, wheezing, noisy breathing.
  • Gastrointestinal issues like abdominal discomfort, stomach/intestinal inflammation, difficulty swallowing, dry mouth, excessive flatulence, pancreatitis, tongue swelling, fecal incontinence, severe constipation, intestinal obstruction, chapped lips.
  • More severe liver enzyme issues like increased GGT level, increased liver enzyme levels.
  • Skin/hair issues like hives, itching, rash, hair loss, eczema, dry skin, skin redness, acne, pus-filled skin lesions, drug rash, skin thickening, dandruff, severe skin rash with blistering and peeling (Stevens-Johnson syndrome or toxic epidermal necrolysis), skin discoloration, dead skin cells at injection site, injection site ulcer.
  • More severe muscle/joint issues like increased creatine phosphokinase, muscle cramps, joint stiffness, muscle weakness, muscle fiber breakdown and muscle pain (rhabdomyolysis), joint swelling.
  • Urinary issues like urinary incontinence, frequent urination, painful urination, urinary retention.
  • Sexual/reproductive issues like erectile dysfunction, ejaculation disorder, menstrual cycle problems (women), breast development in men, sexual dysfunction, breast pain, milk secretion, vaginal discharge, priapism (prolonged erection needing surgery).
  • Swelling of the face, mouth, eyes, or lips, swelling of the body, arms, or legs, redness of the face and neck.
  • General symptoms like increased body temperature, changes in walking, chest pain, discomfort in the chest, malaise, skin hardening, falls, drug withdrawal syndrome.
  • More severe injection site reactions like abscess at injection site, deep skin infection, cyst at injection site, bruise at injection site.

Important Warnings

  • Do not use this medicine if you are allergic to paliperidone, risperidone, or any other ingredient.
  • Tell your doctor immediately if you develop a blood clot, especially in your legs (symptoms include swelling, pain, and redness in the leg, or chest pain and breathing difficulties).
  • Seek immediate medical attention if you have dementia and experience sudden changes in your mental state, or sudden weakness/numbness of the face, arms, or legs (signs of a stroke).
  • Tell your doctor immediately if you have fever, stiff muscles, sweating, or decreased consciousness (called Neuroleptic Malignant Syndrome).
  • If you are a man and have a prolonged or painful erection, tell your doctor immediately.

  • Tell your doctor immediately if you have rhythmic, involuntary movements of your tongue, mouth, or face.
  • Contact your doctor immediately if you experience a severe allergic reaction, such as fever, swelling of the mouth, face, lips, or tongue, shortness of breath, itching, or rash.
  • If you are scheduled for eye surgery (e.g., for cataracts), tell your eye doctor that you are using this medicine.
  • Tell your doctor if you know you have dangerously low levels of white blood cells needed to fight infections.
  • This medicine can cause weight gain. Your doctor will regularly monitor your weight.
  • This medicine can cause or worsen diabetes. Your doctor will monitor your blood sugar levels.
  • Do not use this medicine if you are pregnant or breastfeeding, unless your doctor tells you to.
  • This medicine can cause dizziness, extreme tiredness, and vision problems. Be careful when driving or using machines.
  • Discuss with your doctor if you have Parkinson's disease, heart disease, epilepsy, kidney or liver problems, or a history of stroke or blood clots, as these conditions may worsen.
