Palarccio is a medicine used to treat advanced kidney cancer by slowing down cancer cell growth and cutting off their blood supply.

How to Use

Dosage

The typical daily dose of Palarccio is 20 mg (two 10 mg capsules) once a day when taken with pembrolizumab, or 18 mg (one 10 mg and two 4 mg capsules) once a day when taken with everolimus. If you have severe liver or kidney problems, the recommended dose is 10 mg (one 10 mg capsule) once a day with everolimus.

Method

Swallow the capsules whole with water, with or without food, around the same time each day. Do not open the capsules.

Important

The maximum recommended daily dose is 20 mg (two 10 mg capsules).

Possible Side Effects

COMMON
  • Low blood platelet count, which can cause bruising and poor wound healing.
  • Decreased white blood cell count.
  • Underactive thyroid (tiredness, weight gain, constipation, feeling cold, dry skin), and changes in thyroid blood tests.
  • Changes in blood tests for low potassium and calcium levels.
  • Changes in blood tests for low magnesium and high cholesterol levels.
  • Reduced appetite or weight loss.
  • Difficulty sleeping.
  • Dizziness.
  • Headache.
  • Bleeding (most often nosebleeds, but also blood in urine, bruises, gum bleeding, or intestinal bleeding).
  • High or low blood pressure.
  • Hoarseness.
  • Nausea, vomiting, constipation, diarrhea, stomach pain, indigestion.
  • Dry, sore or inflamed mouth, altered taste.
  • High levels of digestive enzymes (lipase and amylase) in blood tests.
  • Changes in liver function blood tests.
  • Redness, soreness, and swelling of the skin on hands and feet (Hand-Foot Syndrome).
  • Rash.
  • Hair loss.
  • Back pain.
  • Joint or muscle pain.
  • High protein in urine tests and urinary tract infections (increased need to urinate and painful urination).
  • Changes in kidney function blood tests and kidney failure.
  • Severe tiredness or weakness.
  • Swelling in the legs.
  • Dehydration.
  • Stroke symptoms like numbness or weakness on one side of the body, severe headache, seizures, confusion, speech difficulties, vision changes, or dizziness.
  • Palpitations (fast, pounding, or fluttering heartbeat).
  • Heart problems or blood clots in the lungs (shortness of breath, chest pain, arm/back/neck/jaw pain, fast/irregular heartbeat, cough, blue lips/fingers, severe tiredness).
  • Anal fistula (a small channel that forms between the anus and the skin around it).
  • Abdominal swelling or gas.
  • Liver failure.
  • Drowsiness, confusion, poor concentration, loss of consciousness (signs of liver failure).
  • Gallbladder inflammation.
  • Dry, thickened, and itchy skin.
  • General feeling of illness.
  • A hole (perforation) in the stomach or intestines.
RARE
  • Painful infection or irritation near the anus.
  • Severe pain in the upper left abdomen, possibly with fever, chills, nausea, and vomiting (spleen infarct).
  • Mini-stroke (transient ischemic attack).
  • Severe breathing difficulties and chest pain due to air leaking from the lungs into the chest, preventing lung expansion.
  • Pancreatitis (inflammation of the pancreas).
  • Colitis (inflammation of the large intestine).
  • Liver damage.
  • Jaw bone damage (osteonecrosis).
  • Problems with wound healing.
  • Decreased production of hormones by the adrenal glands.
  • Tumor Lysis Syndrome (TLS).
  • Enlargement and weakening of a blood vessel wall or rupture of a blood vessel wall (aneurysms and arterial dissections).
  • Other types of fistulas (abnormal connections between different organs in the body or from an organ to the skin).

Important Warnings

  • Do not take Palarccio if you are allergic to lenvatinib or any other ingredient in this medicine.
  • Do not use Palarccio if you are pregnant, planning to become pregnant, or breastfeeding, as it can harm your baby. Use effective birth control during treatment and for one month after.
  • Talk to your doctor if you have high blood pressure, as Palarccio can make it worse.
  • Tell your doctor if you have ever had heart problems, a stroke, or liver or kidney issues.
  • Inform your doctor if you have recently had surgery, radiation therapy, or plan to have a major operation, as Palarccio can affect wound healing.

  • Report any pain, swelling, numbness, or loose teeth in your mouth or jaw, as these could be signs of jaw bone damage.
  • Palarccio can make you feel dizzy or tired. Do not drive or operate machinery if you experience these effects.
  • Palarccio is not recommended for children and adolescents under 18 years old.
