How to Use

Dosage

The doctor or nurse will start with a small dose, usually 0.2 micrograms per kg per minute, and slowly increase it every 3-5 minutes until your blood pressure reaches the desired level. Doses typically range from 0.2 to 10 micrograms per kg per minute.

Method

Nipruss is given as an infusion directly into a vein, using a special pump. This is always done by a doctor or nurse in a hospital setting.

Important

For controlled blood pressure lowering during surgery, the total dose should not exceed 1.0 to 1.5 mg per kg of body weight. For longer treatments, higher doses might be used, but this is carefully monitored.

Possible Side Effects

COMMON
  • Feeling weak or dizzy
  • Nausea or vomiting
  • Increased heart rate (palpitations)
RARE
  • Reactions at the injection site, such as pain, skin redness, or itching
  • A sudden and significant drop in blood pressure
  • Diarrhea or inability to control bladder (incontinence)
  • Ringing in the ears (tinnitus)
  • Confusion or hallucinations
  • Temporary narrowing of pupils
  • Increased reflexes
  • Temporary worsening of original disease symptoms (rebound effect)
  • Symptoms of cyanide poisoning, such as bright red blood in veins, shallow breathing, headache, irregular heartbeats, body acid imbalance, coma, or seizures. This is serious and needs immediate medical attention.
  • Symptoms of thiocyanate poisoning, especially with kidney problems, including dizziness, headache, loss of appetite, sleep problems, nervousness, low thyroid function, psychosis, paralysis, or coma.

Important Warnings

  • Do not use Nipruss if you are allergic to sodium nitroprusside dihydrate.
  • Tell your doctor if you have taken medicines for erection problems (like sildenafil) or for high blood pressure in the lungs, as Nipruss can cause a dangerous drop in blood pressure.
  • Nipruss is not for children or teenagers.
  • You will always be given another medicine called sodium thiosulfate at the same time as Nipruss to help prevent side effects.
  • Your heart and blood flow will be continuously watched by a doctor or nurse during treatment.

  • Tell your doctor if you have kidney or liver problems, as special tests might be needed to check for harmful substances.
  • Tell your doctor if you have certain serious health problems like specific heart conditions (e.g., narrowed aorta), inherited eye diseases, low vitamin B12, too much acid in your blood, or an underactive thyroid.
