How to Use

Dosage

The usual starting dose is 15 mg or 30 mg daily. Your doctor may increase your dose to 15-45 mg daily. The dose can be adjusted for older adults or those with liver or kidney problems.

Method

Take Mirtazapin ratiopharm at the same time each day. It's usually taken once a day, just before bedtime. Your doctor might suggest splitting your dose, taking one part in the morning and the larger part before bedtime. Swallow the tablets whole with some water or juice, do not chew them.

Important

The maximum daily dose is 45 mg.

Possible Side Effects

COMMON
  • Increased appetite and weight gain
  • Drowsiness or sleepiness
  • Headache
  • Dry mouth
  • Lethargy (feeling of sluggishness)
  • Dizziness
  • Shaking or tremors
  • Nausea
  • Diarrhea
  • Vomiting
  • Constipation
  • Skin rash or other skin reactions
  • Joint pain or muscle pain
  • Back pain
  • Dizziness or feeling faint when standing up quickly (low blood pressure)
  • Swelling (mainly ankles and feet) due to fluid retention
  • Tiredness
  • Vivid dreams
  • Confusion
  • Anxiety
  • Sleep problems
  • Memory problems, which usually disappear after stopping the medicine
RARE
  • Abnormally elevated mood (mania)
  • Yellowing of the eyes or skin (jaundice), which may be a sign of liver problems.
  • Signs of infection such as sudden unexplained high fever, sore throat, and mouth sores (agranulocytosis).
  • Epileptic seizure (convulsion)
  • Serotonin syndrome: symptoms like unexplained fever, sweating, fast heart rate, diarrhea, uncontrollable muscle spasms, shivering, overactive reflexes, restlessness, mood changes, unconsciousness, and increased saliva production.
  • Thoughts of self-harm or suicide
  • Severe skin reactions such as Stevens-Johnson syndrome (red target-like spots, blisters, skin peeling) or Toxic Epidermal Necrolysis.
  • DRESS syndrome (widespread rash, high fever, and swollen lymph nodes).
  • Abnormal skin sensations, such as burning, stinging, itching, or tingling (paresthesia)
  • Restless legs
  • Fainting (syncope)
  • Numbness in the mouth (oral hypesthesia)
  • Low blood pressure
  • Nightmares
  • Agitated state
  • Hallucinations
  • Strong urge to move (akathisia)
  • Muscle jerks or spasms (myoclonus)
  • Aggressiveness
  • Stomach pain and nausea, which may indicate inflammation of the pancreas (pancreatitis)
  • Abnormal sensations in the mouth (oral paresthesia)
  • Swelling of the mouth (oral edema)
  • Whole body swelling (generalized edema)
  • Local swelling
  • Low sodium levels in the blood, which can cause headache, tiredness, seizures, and coma (hyponatremia)
  • Inappropriate antidiuretic hormone secretion
  • Sleepwalking
  • Speech problems
  • Increased creatine kinase levels in the blood
  • Difficulty urinating (urinary retention)
  • Muscle pain, stiffness, and/or weakness, with dark or discolored urine (rhabdomyolysis)
  • Increased prolactin hormone levels in the blood (hyperprolactinemia), with symptoms like enlarged breasts and/or milky discharge from the breasts.
  • Prolonged and painful erection.

Important Warnings

  • Do not take this medicine if you are allergic to mirtazapine or any of its other ingredients.
  • Do not take this medicine if you are currently taking or have recently taken (within the last two weeks) other medicines called MAO inhibitors.
  • If you develop a severe skin rash, skin peeling, blisters, or mouth sores while taking mirtazapine, stop taking the medicine and see a doctor right away. If you have ever had a severe skin reaction to mirtazapine, you should not take it again.
  • If you have thoughts of harming yourself or suicide, contact your doctor or go to a hospital right away.
  • Do not drink alcohol while taking this medicine as it can make you drowsy.

  • If you are pregnant, planning to become pregnant, or breastfeeding, ask your doctor or pharmacist for advice before taking this medicine.
  • This medicine can affect your alertness and concentration. Do not drive or operate machinery until you are sure your abilities are not impaired.
  • Tell your doctor if you have or have ever had epilepsy, liver or kidney problems, heart disease, low blood pressure, schizophrenia, bipolar disorder, diabetes, glaucoma, difficulty urinating due to an enlarged prostate, or certain heart conditions that affect heart rhythm.
  • If you get symptoms of infection like high fever, sore throat, or mouth sores that cannot be explained, stop taking the medicine and see a doctor right away for a blood test.
  • If you are elderly, you might be more sensitive to the side effects of antidepressants.
