Midive widens the blood vessels leading to the lungs, helping the heart pump blood more easily and improving physical ability for patients with certain types of pulmonary hypertension.

How to Use

Dosage

Usually one 1 mg tablet three times a day for two weeks. Your doctor may then slowly increase the dose up to 2.5 mg three times a day.

Method

Take tablets three times a day, about 6–8 hours apart. You can take them with or without food. If you have trouble swallowing, you can crush the tablet and mix it with water or soft food like applesauce just before taking it.

Important

The highest daily dose is 7.5 mg, which means 2.5 mg three times a day.

Possible Side Effects

COMMON
  • Headache
  • Dizziness
  • Indigestion or stomach discomfort after meals
  • Swelling of arms and legs
  • Diarrhea
  • Nausea or vomiting
  • Coughing up blood
  • Stomach inflammation (gastritis)
  • Intestinal inflammation (gastroenteritis)
  • Anemia (low red blood cells, which can make you look pale, feel weak, or short of breath)
  • Palpitations (feeling your heart beat fast or irregularly)
  • Low blood pressure
  • Nosebleed
  • Nasal congestion (difficulty breathing through the nose)
  • Pain in the stomach, gut, or abdominal area
  • Heartburn (reflux disease)
  • Difficulty swallowing
  • Constipation
  • Abdominal bloating (swollen belly)
RARE
  • Sudden bleeding in the lungs, which can be life-threatening.

Important Warnings

  • Do not take if you are allergic to riociguat or any other ingredient in this medicine.
  • Do not take if you are using PDE5 inhibitors (like sildenafil) for high blood pressure or erectile dysfunction.
  • Do not take if you have severe liver problems.
  • Do not take if you are pregnant, think you might be pregnant, or are planning to have a baby. Talk to your doctor.
  • Do not take if you are using nitrates or nitric oxide donors for chest pain or heart disease. This includes 'poppers'.

  • Do not take if you are using other soluble guanylate cyclase stimulators similar to Midive.
  • Do not start if you have low blood pressure (below 90 mmHg for children 6-12 years, below 95 mmHg for patients over 12 years).
  • Do not take if you have high blood pressure in your lungs due to lung scarring of unknown cause.
  • Tell your doctor if you have recently had severe bleeding in your lungs.
  • Tell your doctor if you are taking blood thinners, as this medicine can increase the risk of bleeding.
  • Tell your doctor if you feel dizzy, faint, or lightheaded, as these could be signs of low blood pressure.
  • Tell your doctor if your kidneys are not working well or if you have moderate liver problems.
  • Tell your doctor if you start or stop smoking during treatment, as your dose may need to be changed.
