Midive is a medicine that widens blood vessels from the heart to the lungs, used to treat specific forms of high blood pressure in the lungs (pulmonary hypertension) in adults and children.

How to Use

Dosage

Start with one 1 mg tablet, 3 times a day, for 2 weeks. Your doctor will adjust your dose every two weeks, up to a maximum of 2.5 mg, 3 times a day.

Method

Take tablets 3 times a day, every 6-8 hours, with or without food. If you have trouble swallowing, you can crush the tablet and mix it with water or soft food (like applesauce) right before taking it.

Important

The maximum daily dose is 7.5 mg (2.5 mg, 3 times a day).

Possible Side Effects

COMMON
  • Headache
  • Dizziness
  • Indigestion (stomach upset after meals)
  • Swelling in arms and legs (peripheral edema)
  • Diarrhea
  • Nausea or vomiting
  • Low blood pressure (especially in children 6-17 years old)
RARE
  • Stomach inflammation (gastritis)
  • Digestive tract inflammation (gastroenteritis)
  • Anemia (low red blood cells, causing pale skin, weakness, or breathlessness)
  • Palpitations (irregular, strong, or fast heartbeats)
  • Low blood pressure (in adults)
  • Nosebleed
  • Nasal congestion (difficulty breathing through the nose)
  • Stomach, intestinal, or abdominal pain
  • Heartburn (reflux disease)
  • Difficulty swallowing
  • Constipation
  • Bloating (abdominal distension)
  • Coughing up blood (hemoptysis)
  • Sudden bleeding in the lungs (pulmonary hemorrhage), which can be fatal.

Important Warnings

  • Do not take if you are allergic to riociguat or any other ingredient in this medicine.
  • Do not take if you are pregnant, think you might be pregnant, or plan to get pregnant; use effective birth control and take monthly pregnancy tests.
  • Do not breastfeed, as this medicine may harm your baby.
  • Do not take with certain other medicines like PDE5 inhibitors (e.g., sildenafil), nitrates, or nitric oxide donors, as this can cause very low blood pressure.
  • Do not take if you have severe liver problems or a specific type of high blood pressure in the lungs with lung scarring (idiopathic pulmonary pneumonia).

  • Tell your doctor if you have recently had severe lung bleeding or been treated for coughing up blood.
  • Tell your doctor if you take blood thinners, as this medicine can increase bleeding risk. Your doctor will check your blood and blood pressure regularly.
  • Tell your doctor if you feel very breathless, as this could be fluid building up in your lungs.
  • Tell your doctor if you have symptoms of low blood pressure (dizziness, weakness, fainting) or are over 65 years old, as you have a higher risk.
  • Tell your doctor if you are on dialysis or have severe kidney problems, or moderate liver problems.
  • Tell your doctor if you start or stop smoking during this treatment, as your dose may need to be adjusted.
