How to Use

Dosage

Adults: Typically 47.5 mg once daily. For children (6-18 years) with high blood pressure, the dose is calculated by weight, usually starting at 0.5 mg/kg once daily, maximum 50 mg.

Method

Take the prolonged-release tablets once daily, preferably with breakfast. Swallow whole or divided, do not chew or crush. Take with at least half a glass of water.

Important

The maximum daily dose for adults is generally 190 mg. For children over 6 years, it should not exceed 200 mg.

Possible Side Effects

COMMON
  • Significant drop in blood pressure, also when standing up from lying down, sometimes with fainting.
  • Tiredness.
  • Slow heart rate (bradycardia), feeling of irregular or strong heartbeats (palpitations).
  • Dizziness, headache.
  • Shortness of breath during exercise.
  • Nausea, stomach pain, diarrhea, constipation.
  • Dizziness when changing body position (sometimes fainting), cold hands and feet.
RARE
  • Temporary worsening of heart failure symptoms, severe drop in blood pressure (cardiogenic shock) during or immediately after a heart attack, certain heart rhythm disorders (1st degree AV block), chest pain.
  • Tingling sensation (paresthesia).
  • Bronchospasms (narrowing of airways).
  • Vomiting.
  • Skin changes, psoriasis-like rashes, increased sweating.
  • Muscle cramps.
  • Weight gain.
  • Fluid retention (edema).
  • Depression, reduced concentration, drowsiness, insomnia, nightmares.
  • Functional heart disorders, abnormal heart rhythm (arrhythmia).
  • Vision problems, dry or irritated eyes, conjunctivitis.
  • Feeling cold.
  • Dry mouth.
  • Hair loss.
  • Abnormal liver function test results.
  • Impotence and other sexual dysfunctions.
  • Nervousness, tension.
  • Reduced number of platelets (thrombocytopenia).
  • Ringing in the ears (tinnitus).
  • Taste disturbances.
  • Photosensitivity, worsening of psoriasis.
  • Joint pain.
  • Tissue death (necrosis) in patients with severe blood circulation problems in arms and/or legs before treatment.
  • Liver inflammation (may cause yellowing of the skin and/or eyes).
  • Forgetfulness or impaired memory, confusion, hallucinations.

Important Warnings

  • Do not take this medicine if you are allergic to metoprolol or other similar medicines (beta-blockers).
  • Do not take this medicine if you have severe heart problems, such as a very slow heartbeat (below 50 beats per minute) or serious issues with your heart's electrical signals.
  • Do not take this medicine if your blood pressure is very low (systolic pressure below 90 mmHg).
  • Do not take this medicine if you have severe asthma or chronic obstructive pulmonary disease (COPD).
  • Do not take this medicine if you have an untreated tumor of the adrenal gland (pheochromocytoma).

  • Do not stop taking this medicine suddenly without talking to your doctor, as it can worsen your heart condition and increase the risk of a heart attack.
  • Tell your doctor if you have diabetes (as this medicine can hide symptoms of low blood sugar), mild heart rhythm problems, chest pain called Prinzmetal's angina, an overactive thyroid, psoriasis, or if you often have strong allergic reactions.
  • Always inform your doctor or the anaesthetist about this medicine before any surgery.

Use in Elderly Patients

Category A

Suitable for older adults. Significant adverse effects include bradycardia and orthostatic hypotension. Consider potential interactions especially with with drugs affecting the conduction system of the heart. (updated 20.1.2025)
