How to Use

Dosage

Your doctor will tell you how much medicine to use, as it's different for everyone.

Method

You will give yourself an injection under the skin once a week, usually on the same day. Your doctor or nurse can teach you how.

Important

This medicine is only taken once a week. Never take more than your doctor prescribes for the week, as it can be very harmful.

Possible Side Effects

COMMON
  • Mouth sores, digestive issues, nausea, loss of appetite, stomach pain.
  • Abnormal liver function test results.
  • Mouth ulcers, diarrhea.
  • Rash, red or itchy skin.
  • Headache, tiredness, drowsiness.
  • Reduced blood cell counts (white, red, or platelets).
  • Persistent dry cough, shortness of breath, and fever (signs of lung inflammation).
RARE
  • Yellowing of the skin and whites of the eyes, signs of liver damage.
  • Rash, red itchy skin, swelling (hands, feet, face, lips, throat), difficulty breathing, feeling weak, signs of severe allergic reaction.
  • Swelling of hands, feet or ankles, changes in urination, reduced or no urine production, signs of kidney failure.
  • Fever, chills, aches, sore throat, signs of serious infections like pneumonia or blood poisoning.
  • One-sided body weakness, pain, swelling, redness, or warmth in one leg, signs of blood clot (stroke, deep vein thrombosis).
  • Fever, general weakness, sore throat, mouth pain, urination problems, rapid decrease in white blood cells, signs of severe bone marrow suppression.
  • Unexpected bleeding (gums, urine, vomit) or bruising, signs of significantly reduced platelet count.
  • Severe headache, fever, stiff neck, nausea, vomiting, confusion, light sensitivity, signs of meningitis.
  • Changes in mental state, movement disorders, vision problems, or memory problems.
  • Severe rash or blistering of the skin, mouth, eyes, or genitals, signs of Stevens-Johnson syndrome or toxic epidermal necrolysis.
  • Sore throat.
  • Intestinal inflammation, vomiting, pancreatitis, black/tarry stools, gastrointestinal ulcers/bleeding.
  • Sunburn-like reactions, increased sun sensitivity, hair loss, skin ulcers, shingles, vasculitis, hives.
  • Onset of diabetes.
  • Dizziness, confusion, depression.
  • Low albumin levels in blood.
  • Bladder/vaginal inflammation or ulcers, impaired kidney function, abnormal urination.
  • Joint pain, muscle pain, reduced bone mass.
  • Gum inflammation.
  • Increased skin pigmentation, acne, small blue spots on skin, allergic blood vessel inflammation.
  • Low antibody levels in blood.
  • Infections (including reactivation of old infections), red eyes (conjunctivitis).
  • Mood swings.
  • Vision problems.
  • Heart inflammation, fluid around the heart, heart compression.
  • Low blood pressure.
  • Lung scarring, shortness of breath, asthma, fluid around lungs.
  • Stress fracture.
  • Electrolyte imbalances.
  • Fever, poor wound healing.
  • Acute toxic colon enlargement.
  • Nail discoloration, nail inflammation, boils, visible small blood vessels.
  • Pain, weakness, numbness, tingling, altered taste (metallic), seizures, paralysis, meningism.
  • Reduced sex drive, impotence, male breast enlargement, abnormal sperm, menstrual disorders, vaginal bleeding.
  • Swollen lymph nodes (lymphoma).
  • Overgrowth of white blood cells (lymphoproliferative disorders).
  • Increased white blood cells.
  • Nosebleeds.
  • Protein in urine.
  • Weakness.
  • Jaw bone damage.
  • Tissue damage at injection site.
  • Skin redness and peeling.
  • Swelling.

Important Warnings

  • Do not use Metoject PEN if you are allergic to methotrexate or any other ingredient.
  • Do not use if you have severe liver, kidney, or blood problems.
  • Do not use if you drink a lot of alcohol often.
  • Do not use if you have a serious infection (like tuberculosis or HIV) or a weak immune system.
  • Do not use if you have sores in your mouth, stomach, or intestines.

  • Do not use if you are pregnant, planning to get pregnant, or breastfeeding.
  • Do not use if you are getting live vaccines.
  • This medicine can affect your ability to have children and can cause birth defects. Women should not get pregnant during treatment and for 6 months after. Men should not father a child during treatment and for 3 months after.
  • If you cough up or spit blood, tell your doctor right away. This could be a sign of bleeding in your lungs.
  • Stay out of strong sunlight and avoid tanning beds, as this medicine can make your skin sensitive. Wear protective clothes and sunscreen.
  • If you have diarrhea, tell your doctor. You may need to stop the treatment.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. For the treatment of conditions other than cancer only once weekly. Organtoxic such as kidneys, liver and bone marrow. Reduce the dose already in mild renal impairment. Avoid use in severe renal impairment. Consider the numerous potential interactions. Remember blood tests to monitor safety. (updated 6.11.2023)
