How to Use

Dosage

Typical dosage varies from 47.5 mg to 190 mg of metoprolol succinate once daily, depending on the condition.

Method

Take tablets once daily, with or without food. You can swallow them whole or divided, but do not chew or crush them. Take with water (at least half a glass).

Important

The maximum dose is 190 mg of metoprolol succinate once daily or the highest tolerated dose as prescribed by your doctor.

Possible Side Effects

COMMON
  • tiredness
  • dizziness, headache
  • slow heart rate (bradycardia)
  • palpitations
  • significant drop in blood pressure, especially when standing up from lying down, rarely with loss of consciousness
  • cold hands and feet
  • shortness of breath during exertion in prone patients (e.g., asthma patients)
  • nausea, stomach pain, diarrhea, constipation
RARE
  • weight gain
  • depression, drowsiness, sleep disturbances, nightmares, impaired concentration
  • abnormal skin tingling, prickling, or numbness (paresthesia)
  • temporary worsening of heart failure symptoms (including swollen ankles and feet), first-degree heart conduction disorder (first-degree AV block), chest pain (angina), weak heart pumping ability (cardiogenic shock) in heart attack patients
  • bronchial spasms (bronchospasm)
  • rash (psoriasis-like hives and skin atrophy), increased sweating
  • muscle cramps
  • vomiting
  • worsening of asymptomatic diabetes (latent diabetes mellitus)
  • nervousness, anxiety
  • vision problems, dry or irritated eyes (note for contact lens users), conjunctivitis
  • heart rhythm disorders, conduction disorders
  • nasal congestion
  • dry mouth
  • abnormal liver function tests
  • hair loss
  • impotence and other libido disorders, Peyronie's disease (hardening of erectile tissue in the penis)
  • low platelet count (thrombocytopenia), low white blood cell count (leukopenia)
  • forgetfulness or memory impairment, confusion, hallucinations, personality changes (e.g., mood swings)
  • ringing in the ears (tinnitus), hearing impairment
  • tissue death (necrosis) in patients with severely impaired peripheral circulation when treatment is started
  • taste disorders
  • liver inflammation
  • photosensitivity (rash after light exposure), worsening or onset of psoriasis
  • joint pain, muscle weakness

Important Warnings

  • Do not use if you are allergic to metoprolol succinate or any other ingredients in this medicine.
  • Avoid if you have severe asthma or severe wheezing attacks.
  • Do not use if you are in shock.
  • Do not use if you have severe heart conduction problems (like certain types of heart block) or rhythm disorders (sick sinus syndrome), unless you have a permanent pacemaker.
  • Do not use if you have uncontrolled heart failure that is not being treated.

  • Do not use if your heart rate is too slow (below 50 beats per minute at rest before treatment).
  • Do not use if you have very low blood pressure (systolic blood pressure below 90 mmHg).
  • Do not stop Metohexal suddenly. The dose must be reduced gradually, as sudden discontinuation can worsen heart failure and increase the risk of heart attack or sudden cardiac death.

Use in Elderly Patients

Category A

Suitable for older adults. Significant adverse effects include bradycardia and orthostatic hypotension. Consider potential interactions especially with with drugs affecting the conduction system of the heart. (updated 20.1.2025)
