How to Use

Dosage

The usual dose for most patients is one 50 mg/12.5 mg tablet once daily.

Method

Swallow the tablets whole with a glass of water. You can take them with or without food.

Important

The maximum daily dose is two 50 mg/12.5 mg tablets or one 100 mg/25 mg tablet.

Possible Side Effects

COMMON
  • Cough, stuffy nose, sinus problems.
  • Diarrhea, stomach pain, nausea, indigestion.
  • Muscle pain or cramps, leg pain, back pain.
  • Headache, dizziness, trouble sleeping.
  • Weakness, tiredness, chest pain.
  • High potassium levels (can cause abnormal heart rhythm), low hemoglobin.
  • Kidney problems or kidney failure, low blood sugar.
RARE
  • Severe allergic reaction (rash, swelling of face/lips/throat/tongue, difficulty breathing/swallowing). Seek immediate medical help.
  • Acute breathing problems (severe shortness of breath, fever, weakness, confusion). Seek immediate medical help.
  • Blood disorders like anemia, low white blood cells, bleeding problems, or unusual skin spots.
  • Metabolic changes (gout, high or low blood sugar, electrolyte imbalance, loss of appetite).
  • Nervous system issues (anxiety, depression, memory problems, sleep disturbances, dizziness, tingling, tremors, fainting, vertigo, taste disorder).
  • Eye problems (blurred vision, eye pain, yellow vision, inflammation, vision loss due to high eye pressure).
  • Ear problems (ringing in the ears).
  • Heart and circulation problems (low blood pressure, chest pain, irregular heartbeat, heart attack, stroke, blood vessel inflammation).
  • Lung and throat problems (sore throat, shortness of breath, lung inflammation, fluid in lungs, nosebleeds, stuffy nose).
  • Stomach and gut problems (severe constipation, gas, abdominal cramps, vomiting, dry mouth, salivary gland inflammation, jaundice, pancreatitis, liver inflammation, intestinal swelling).
  • Skin problems (hives, itching, rash, redness, sun sensitivity, hair loss, skin and lip cancer).
  • Joint and muscle problems (pain, swelling, stiffness, muscle weakness, unexplained muscle pain with dark urine).
  • Urinary and sexual problems (frequent urination, urinary tract infection, sugar in urine, decreased sexual desire, impotence).
  • General symptoms like fever or flu-like illness.

Important Warnings

  • Do not take if you are allergic to losartan, hydrochlorothiazide, or any other ingredients in this medicine.
  • Do not take if you are allergic to other similar medicines called sulfonamides (e.g., some antibiotics).
  • Do not take if you have severe liver problems, severe kidney problems, or if your kidneys do not make urine.
  • Do not take if you are more than 3 months pregnant, as it can seriously harm your baby. Avoid use during early pregnancy too.
  • Do not take if you have gout or if your potassium or sodium levels are too low, or calcium levels are too high, and not corrected by treatment.

  • Tell your doctor immediately if you get severe shortness of breath or trouble breathing, especially if you've had lung problems with hydrochlorothiazide before.
  • Contact your doctor if you get vision loss or eye pain. This could be a sign of serious eye problems that can lead to permanent vision loss if not treated. The risk is higher if you have had penicillin or sulfonamide allergy.
  • Do not drink alcohol or grapefruit juice while taking this medicine. Alcohol can increase its effects, and grapefruit juice can reduce its effectiveness.
  • Be careful when driving or using machines until you know how this medicine affects you.
  • Tell your doctor if you are taking other blood pressure medicines like ACE inhibitors (e.g., enalapril) or aliskiren, especially if you have diabetes and kidney problems.
  • Protect your skin from sun and UV light if you have had skin cancer or develop new skin changes, as hydrochlorothiazide can increase the risk of certain skin cancers with long-term high-dose use.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Effectiveness is reduced in renal impairment. Consider risk of orthostatic hypotension and hyponatraemia and -kalemia. (updated 19.5.2025)
