How to Use

Dosage

For high blood pressure, the starting dose is usually 5 mg to 20 mg once a day. For heart failure, the starting dose is 2.5 mg once a day, gradually increasing over 2-4 weeks to a usual dose of 20 mg daily, taken once or twice a day.

Method

Always take this medicine by mouth, with or without food.

Important

The maximum daily dose is 40 mg.

  • Runny nose, sore throat, breathing difficulties, or asthma
  • Inflammation of the pancreas, vomiting, indigestion, constipation, loss of appetite, or dry mouth
  • Hives or hair loss
  • Impotence or muscle cramps
  • Kidney problems
  • Yellow skin or eyes (jaundice)
  • Allergic swelling in the bowel (causing abdominal pain, nausea, vomiting)
  • Flu-like symptoms (fever, body aches, rash, sensitivity to sunlight)

Important Warnings

  • Stop taking immediately and seek medical help if you experience signs of a severe allergic reaction, such as swelling of your hands, face, lips, tongue or throat, difficulty swallowing or breathing, itching, or hives.
  • Stop taking immediately and see a doctor if you feel very dizzy or faint, especially when starting treatment or if your dose has been increased, or if you develop severe skin redness, blisters, or peeling.
  • Do not take this medicine if you are more than 3 months pregnant, as it can harm your baby. It is also best to avoid it during early pregnancy.
  • Do not take this medicine if you are taking or have recently taken sacubitril/valsartan or if you have diabetes/kidney problems and are taking aliskiren.
  • Limit alcohol intake, as it can cause your blood pressure to drop too much, leading to dizziness or fainting.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose already in mild renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
