How to Use

Dosage

Start with one 25 mg tablet once a day. After about 4 weeks, the dose is usually increased to one 50 mg tablet (or two 25 mg tablets) once a day.

Method

You can take Inspra with or without food. Swallow the tablet whole with plenty of water.

Important

The maximum daily dose is 50 mg.

Possible Side Effects

COMMON
  • High potassium levels in the blood (symptoms may include muscle cramps, diarrhea, nausea, dizziness, or headache)
  • Fainting
  • Dizziness
  • Increased cholesterol levels in the blood
  • Difficulty sleeping (insomnia)
  • Headache
  • Heart problems, such as irregular heartbeat or worsening heart failure
  • Cough
  • Constipation
  • Low blood pressure
  • Diarrhea
  • Nausea
  • Vomiting
  • Abnormal kidney function
  • Rash
  • Itching
  • Back pain
  • Feeling weak
  • Muscle cramps
  • Increased urea in the blood
  • Increased creatinine in the blood (which may indicate kidney problems)
RARE
  • Swelling of the face, tongue, or throat; difficulty swallowing; hives and trouble breathing (signs of a severe allergic reaction)
  • Infection
  • Eosinophilia (an increase in certain white blood cells)
  • Decreased sodium levels in the blood
  • Dehydration
  • Increased triglyceride levels (fats) in the blood
  • Rapid heartbeat
  • Gallbladder inflammation
  • Drop in blood pressure, which may cause dizziness when standing up
  • Blood clot in the leg
  • Sore throat
  • Flatulence (gas)
  • Underactive thyroid gland
  • Increased blood sugar levels
  • Decreased sense of touch
  • Increased sweating
  • Musculoskeletal pain
  • General feeling of illness (malaise)
  • Kidney inflammation
  • Male breast enlargement
  • Changes in some blood test results

Important Warnings

  • Do not take if you are allergic to eplerenone or any other ingredient in this medicine.
  • Do not take if your blood potassium levels are high.
  • Do not take if you are using potassium-sparing diuretics (water pills).
  • Do not take if you have severe kidney or liver disease.
  • Do not take with certain antifungal medications (ketoconazole, itraconazole), HIV medications (nelfinavir, ritonavir), antibiotics (clarithromycin, telithromycin), or nefazodone (for depression).

  • Do not take with certain heart medications or high blood pressure medicines (ACE inhibitors and ARBs) at the same time.
  • If you feel dizzy after taking Inspra, do not drive or use machines.
  • This medicine contains lactose; consult your doctor if you have a sugar intolerance.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe and severe renal impairment. Consider risk of hyperkalaemia in patiets with impaired renal function. Avoid grapefruit. (updated 10.11.2025)
