How to Use

Dosage

Start with one 25 mg tablet once daily. After about 4 weeks, your doctor may increase the dose to one 50 mg tablet once daily (or two 25 mg tablets).

Method

Swallow the tablet whole with a good amount of water. You can take it with or without food.

Important

The highest daily dose is 50 mg.

Possible Side Effects

COMMON
  • High potassium in blood (may cause muscle cramps, diarrhea, nausea, dizziness, or headache)
  • Fainting
  • Dizziness
  • High cholesterol levels
  • Trouble sleeping (insomnia)
  • Headache
  • Heart problems (such as irregular heartbeat or worsening heart failure)
  • Cough
  • Constipation
  • Low blood pressure
  • Diarrhea
  • Nausea
  • Vomiting
  • Kidney problems (shown in blood tests)
  • Rash
  • Itching
  • Back pain
  • Feeling weak
  • Muscle spasms
RARE
  • Serious allergic reaction with swelling of the face, tongue, or throat, trouble swallowing, hives, or difficulty breathing. Seek medical help right away.
  • Infection
  • Increase in certain white blood cells (eosinophilia)
  • Low sodium in blood
  • Dehydration
  • High triglyceride (fat) levels in blood
  • Fast heartbeat
  • Inflammation of the gallbladder
  • Low blood pressure when standing up (may cause dizziness)
  • Blood clot in the leg
  • Sore throat
  • Gas in the stomach or intestines
  • Underactive thyroid gland
  • High blood sugar
  • Reduced sense of touch
  • Increased sweating
  • Pain in muscles and bones
  • General feeling of being unwell
  • Kidney inflammation
  • Breast enlargement in men
  • Changes in some blood test results

Important Warnings

  • Do not take Inspra if you are allergic to eplerenone or any other ingredients.
  • Do not take Inspra if you have high potassium levels in your blood.
  • Do not take Inspra if you are taking other 'water pills' that save potassium (potassium-sparing diuretics).
  • Do not take Inspra if you have severe kidney or liver problems.
  • Do not take Inspra if you are taking certain medicines for fungal infections (ketoconazole, itraconazole), HIV (nelfinavir, ritonavir), bacterial infections (clarithromycin, telithromycin), or depression (nefazodone).

  • Do not take Inspra if you are already taking certain other heart or blood pressure medicines called ACE inhibitors and ARBs together.
  • Inspra contains lactose (a type of sugar). If your doctor has told you that you cannot tolerate some sugars, talk to your doctor before taking this medicine.
  • This medicine may make you feel dizzy. If you feel dizzy, do not drive or use machines.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe and severe renal impairment. Consider risk of hyperkalaemia in patiets with impaired renal function. Avoid grapefruit. (updated 10.11.2025)
