How to Use

Dosage

Your doctor will decide your correct dose.

Method

You will take this medicine as an injection under the skin only once a week. Your doctor or nurse will give the injection or teach you how to give it yourself.

Important

This medicine should be taken no more than once a week. Taking too much can be life-threatening.

Possible Side Effects

COMMON
  • Inflammation in the mouth lining, indigestion, nausea, loss of appetite, stomach pain.
  • Abnormal liver function tests.
RARE
  • Persistent dry cough, shortness of breath, and fever (could be lung inflammation).
  • Spitting or coughing up blood (signs of lung bleeding).
  • Symptoms of liver damage, like yellow skin or eyes (can be cirrhosis, fibrosis, fatty liver, hepatitis, or liver failure).
  • Allergic reactions such as red, itchy rash; swelling of hands, feet, ankles, face, lips, mouth, or throat (may cause difficulty swallowing or breathing); feeling faint (can be severe allergic reaction or anaphylactic shock).
  • Symptoms of kidney damage, like swelling of hands, ankles, or feet; changes in how often you urinate, or reduced/no urine (signs of kidney failure).
  • Signs of infection like fever, chills, body aches, sore throat (can be severe infections like Pneumocystis jirovecii pneumonia or sepsis).
  • Symptoms like weakness on one side of the body (stroke), or pain, swelling, redness, and unusual warmth in one leg (deep vein thrombosis).
  • Blood clot that blocks a blood vessel.
  • Fever and severe worsening of general health, or sudden fever with sore throat/mouth or urination problems (due to low white blood cells or bone marrow problems).
  • Unexpected bleeding, e.g., from gums, blood in urine, vomiting blood, or bruises (due to low platelet count or bone marrow problems).
  • Severe headache, often with fever, stiff neck, feeling unwell, vomiting, confusion, and light sensitivity (signs of brain lining inflammation).
  • Brain disorders with changes in mental state, movement problems (ataxia), vision problems, or memory issues.
  • Severe skin rash or blisters (may also appear in mouth, eyes, or genitals), possibly Stevens-Johnson syndrome or toxic epidermal necrolysis.
  • Mouth sores, diarrhea.
  • Rash, skin redness, itching.
  • Headache, tiredness, drowsiness.
  • Reduced blood cell formation (white/red blood cells, platelets).
  • Sore throat.
  • Intestinal inflammation, vomiting, pancreatitis, black or tarry stools, stomach/intestinal ulcers and bleeding.
  • Sunburn-like reactions, increased sensitivity to sunlight, hair loss, increased rheumatic nodules, skin ulcers, shingles, vasculitis, herpes-like rash, hives.
  • Onset of adult-onset diabetes.
  • Dizziness, confusion, depression.
  • Decreased serum albumin.
  • Decrease in all blood cells and platelets.
  • Bladder or vaginal inflammation and ulcers, kidney function impairment, urination problems.
  • Joint pain, muscle pain, decreased bone density.
  • Gum inflammation.
  • Increased skin pigmentation, acne, blue spots on skin due to bleeding (bruises, petechiae), allergic vasculitis.
  • Decreased antibodies in blood.
  • Infection (including reactivation of inactive chronic infection), eye inflammation (conjunctivitis).
  • Mood swings.
  • Vision problems.
  • Inflammation of the heart sac, fluid buildup around the heart, compression of the heart due to fluid.
  • Low blood pressure.
  • Scarring of lung tissue (lung fibrosis), shortness of breath, asthma, fluid in lung lining.
  • Stress fracture.
  • Electrolyte imbalances.
  • Fever, poor wound healing.
  • Acute toxic enlargement of the colon (toxic megacolon).
  • Increased nail pigmentation, nail fold inflammation (acute paronychia), deep hair follicle infection (boils), visible widening of small blood vessels.
  • Local injection site injury (sterile abscesses, fat tissue changes) after injection into muscle or under skin.
  • Pain, loss of strength or sensation, numbness or tingling / reduced response to stimulation, taste changes (metallic taste), convulsions, paralysis, meningism.
  • Vision loss, non-inflammatory eye disease (retinopathy).
  • Loss of sex drive, impotence, male breast enlargement, impaired sperm production (oligospermia), menstrual disorders, vaginal discharge.
  • Swollen lymph nodes (lymphoma).
  • Lymphoproliferative disorders (abnormal growth of white blood cells).
  • Increased number of certain white blood cells.
  • Nosebleeds.
  • Protein in urine.
  • Feeling of weakness.
  • Jaw bone damage (due to overgrowth of white blood cells).
  • Skin redness and peeling.
  • Swelling.
  • Tissue damage at injection site.

Important Warnings

  • Do not use if you are allergic to methotrexate or any other ingredient in this medicine.
  • Do not use if you have severe liver or kidney disease, or a serious blood disorder.
  • Avoid large amounts of alcohol, coffee, caffeine-containing soft drinks, and black tea during treatment.
  • Do not use if you have a severe infection (like tuberculosis, HIV), or an immune system problem.
  • Do not use if you have mouth sores or stomach/intestinal ulcers.

  • Avoid live vaccines while taking this medicine.
  • Do not use if you are pregnant or breastfeeding. Methotrexate can cause birth defects and miscarriage. Women should avoid getting pregnant during treatment and for 6 months after. Men should avoid fathering a child during treatment and for 3 months after.
  • This medicine can make your skin more sensitive to sunlight. Avoid strong sunlight and do not use a solarium without medical advice.
  • If you have diarrhea, contact your doctor immediately, as treatment may need to be stopped.
  • If you notice new or worsening neurological symptoms like muscle weakness, vision problems, confusion, or changes in thinking, contact your doctor right away.
  • If you cough up blood or spit blood, contact your doctor immediately.
  • Injexate is not recommended for children under 3 years old.
  • This medicine can cause tiredness and dizziness. Do not drive or operate machines if you feel drowsy.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. For the treatment of conditions other than cancer only once weekly. Organtoxic such as kidneys, liver and bone marrow. Reduce the dose already in mild renal impairment. Avoid use in severe renal impairment. Consider the numerous potential interactions. Remember blood tests to monitor safety. (updated 6.11.2023)
