How to Use

Dosage

Your doctor or nurse will decide your dose based on your pain, previous pain medications, age, and weight. Typically, 1-2 mg as an injection into a vein or under the skin, or 0.15-0.45 mg per hour as an infusion.

Method

A doctor or nurse will give you this medicine as an injection into a vein (intravenously) or under the skin (subcutaneously), or as an infusion. It can also be given via a pain pump.

Important

Your doctor will adjust the dosage until your pain is relieved; there is no fixed maximum daily dose as it is determined by your doctor.

Possible Side Effects

COMMON
  • Dizziness or strong drowsiness
  • Constipation
  • Nausea or vomiting
  • Itchy skin, rash, or sweating
  • Unusual weakness or fatigue
  • Confusion, anxiety, or hallucinations
  • Low blood pressure or rapid heart rate
  • Dry mouth
  • Difficulty urinating or urinary urgency
  • Loss of appetite
  • Abdominal pain or stomach discomfort
  • Skin reactions at the injection site
RARE
  • Headache, tremors, muscle twitching, or tingling in hands or feet
  • Pupil constriction or vision problems
  • Digestive problems, diarrhea, or taste disturbances
  • Decreased libido or impotence
  • Development of drug tolerance or withdrawal symptoms (e.g., agitation, anxiety, insomnia, shaking)
  • Depression or nightmares
  • Shortness of breath, wheezing, slow or irregular heart rate, or sleep apnea (breathing pauses during sleep)
  • Drug dependence, sedation, or seizures
  • Increased pain sensitivity (hyperalgesia)
  • Bowel dysfunction (paralytic ileus)
  • Swelling in hands or feet, skin irritation or hardening at the injection site
  • Aggression or hot flashes
  • Withdrawal symptoms in newborn babies if the mother used the medicine during pregnancy
  • Changes in liver or pancreas blood test results

Important Warnings

  • Do not use if you are allergic to hydromorphone or any other ingredients in this medicine.
  • Do not use if you have severe breathing difficulties or serious lung diseases.
  • Do not use if you have severe stomach pain or gut problems where your intestines do not move.
  • Do not use if you have taken certain antidepressant medicines (MAOIs) in the last two weeks.
  • Tell your doctor if you have a head injury, seizures, or alcohol dependence.

  • Tell your doctor if you have problems with your lungs, heart, kidneys, or liver.
  • Tell your doctor if you have a history of drug abuse or addiction.
  • This medicine is not recommended for children under 12 years of age.
  • Be careful with other medicines, especially sedatives, sleeping pills, or medicines for anxiety, epilepsy, or nerve pain, as they can cause very serious breathing problems or coma.
  • Do not stop taking this medicine suddenly without talking to your doctor, as it can cause withdrawal symptoms.
  • Avoid alcohol while taking this medicine, as it can increase drowsiness.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Sedative. Reduce the initial dose in moderately severe renal impairment. Increases the risk of confusion and falling. Need for laxative is often necessary. (updated 24.3.2025)
