How to Use

Dosage

Your doctor will determine your individual dose. For children, doses are based on weight and age; for example, 150 IU/kg for infants (1 month to under 2 years), 125 IU/kg for children (2 to under 8 years), and 100 IU/kg for older children (8 to under 18 years), usually given twice daily.

Method

Fragmin is usually given as an injection under the skin (subcutaneously). In some cases, such as in a hospital or for blood purification treatments like hemodialysis, it may be given into a vein.

Important

Your doctor will determine the maximum daily dose for you based on your condition and individual needs.

Possible Side Effects

COMMON
  • Mild decrease in platelets (blood clotting cells), which usually gets better during treatment
  • High potassium levels in the blood, especially for those with kidney problems or diabetes
  • Bleeding
  • Temporary increase in liver enzymes
  • Bruising or pain at the injection site
RARE
  • Allergic reaction
  • Local skin damage (necrosis)
  • Temporary hair loss
  • Rash
  • Severe allergic reactions (anaphylaxis)
  • Severe decrease in platelets due to heparin
  • Bleeding inside the head (intracranial hemorrhage)
  • Bleeding inside the abdomen (retroperitoneal hemorrhage)
  • Bleeding around the spine or brain (epidural or spinal hematoma)

Important Warnings

  • Do not use if you are allergic to dalteparin, heparin, or any other ingredients in this medicine.
  • Do not use if you have had a low platelet count diagnosed by a doctor.
  • Do not use if you have a stomach or duodenal ulcer.
  • Do not use if you have or have had a brain hemorrhage or any other active bleeding condition.
  • Do not use if you have an increased tendency to bleed.

  • Do not use if you have or have had a sudden bacterial infection of the heart (septic endocarditis).
  • Do not use if you have had a recent injury or surgery to your brain, eyes, or ears.
  • Fragmin should not be used in newborns younger than 1 month.
  • Tell your doctor if you are going to have a spinal or epidural injection, as large doses of Fragmin might increase the risk of bleeding.

Category C
